应激性溃疡(stress ulceration),即急性胃粘膜病变(acute gastric mucosal lesion,AGML),是颅脑外伤、严重烧伤的常见并发症之一,脑外伤后发生率在20%左右。一般认为应激性溃疡合并上消化道出血与颅脑损伤的严重程度、年龄等有密切相关性,而应激性溃疡与颅内血肿类型的关系尚不明确。1988年5月~1996年5月期间,我科收治了484例颅内血肿病人,所有病人接受了手术治疗,其中外伤性颅内血肿(TICH)368例(76%);自发性颅内血肿(SICH)116例(24%),共70例并发了应激性溃疡,总发病率为14.5%,详细报告如下。
Stress ulceration, acute gastric mucosal lesion (AGML), is one of the common complications of craniocerebral injury and severe burn. The incidence of traumatic brain injury is about 20%. It is generally believed that stress ulcer associated with upper gastrointestinal bleeding and the severity of brain injury, age, etc. are closely related to the relationship between stress ulcer and the type of intracranial hematoma is not yet clear. From May 1988 to May 1996, 484 patients with intracranial hematomas were treated in our department, and all the patients underwent surgical treatment, including 368 cases (76%) of traumatic intracranial hematomas (TICH), 34 cases of spontaneous intracranial hematoma (SICH) 116 cases (24%), a total of 70 cases complicated by stress ulcer, the overall incidence was 14.5%, the detailed report is as follows.
