目的:观察中西医结合治疗一氧化碳中毒患者的疗效以及对心肌酶谱指标和甲襞微循环指标的影响。方法:选择142例一氧化碳中毒患者,按照随机数字表法分为观察组与对照组各71例。对照组采用西医疗法治疗,包括高压氧、吸氧、胞二磷胆碱、舒血宁注射液,观察组在对照组治疗基础上加用活血养心汤口服治疗,7天为1疗程,治疗3疗程。观察2组患者的临床治疗效果,统计分析治疗前后心肌酶谱指标[肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、肌钙蛋白I(cTnI)、缺血修饰白蛋白(IMA)]及甲襞微循环指标(管襻积分、襻周状态积分、管襻畸形率、白微栓率)的变化。结果:临床疗效总有效率观察组95.77%,对照组76.06%,2组比较,差异有统计学意义(P<0.01)。治疗前,2组心肌酶谱指标、甲襞微循环指标比较,差异均无统计学意义(P>0.05)。治疗后,2组CK、CK-MB、管襻积分、襻周状态积分、管襻畸形率、白微栓率降低,cTnI、IMA升高,差异均有统计学意义(P<0.05);观察组cTnI、IMA、管襻积分、襻周状态积分、管襻畸形率、白微栓率与对照组比较,差异均有统计学意义(P<0.05),2组CK、CK-MB比较,差异均无统计学意义(P>0.05)。结论:采用中西医结合治疗一氧化碳中毒患者具有确切的临床疗效,对心肌具有一定的保护作用,利于患者的预后。
Objective: To observe the curative effect of combining traditional Chinese and western medicine on patients with carbon monoxide poisoning and its influence on myocardial enzymes and microcirculation index of nailfolds. Methods: 142 patients with carbon monoxide poisoning were selected and randomly divided into observation group and control group according to random number table method. The control group was treated with Western medical therapy, including hyperbaric oxygen, oxygen inhalation, Citicoline and Shuxuening injection. The observation group was treated with Huoxue Yangxin Tang on the basis of the control group. One course of treatment was given for 7 days. The treatment group 3 courses. The clinical effects of two groups of patients were observed. The changes of myocardial enzymes such as creatine kinase (CK), creatine kinase (CK-MB), troponin I (cTnI), ischemia-modified albumin (IMA)], and microcirculation indexes of Nailfold (tubal score, periocular state score, duct deformity rate and white micro-embolism rate). Results: The total effective rate of clinical observation group was 95.77% in the control group and 76.06% in the observation group. The difference was statistically significant (P <0.01). Before treatment, there were no significant differences between the two groups in myocardial enzymes and formazan microcirculation (P> 0.05). After treatment, the levels of cTnI, cTnI and IMA in two groups were significantly increased (P <0.05) after treatment. The scores of CK, CK-MB, (P <0.05). There was no significant difference in CK, CK-MB and CK between the two groups Significance (P> 0.05). Conclusion: The treatment of carbon monoxide poisoning in patients with Integrative Medicine has the exact clinical efficacy, has a certain protective effect on myocardial, which will benefit the prognosis of patients.
