颅外椎动脉瘤罕见,文献报道很少。本文报告一例。20岁士兵,因颈部左后外方有一搏动性肿块1月入院。入院前3月,患者颈部弹片伤,由于大出血而于后送中心结扎了左颈动脉。疑为左椎动脉创伤性动脉瘤。行左侧椎动脉造影显示C_2水平有一巨大动脉瘤。左侧大脑中动脉通过开放的后交通动脉得到充盈。血管造影发现半圆形块物并提示有左颈动脉闭塞。用自体静脉移植重建椎动脉。作颈部超过1/3的左后正中旁切口,分离椎动脉远端,磨去
Extracranial vertebral aneurysms are rare and rarely reported in the literature. This article reports an example. A 20-year-old soldier admitted to hospital with a pulsatile mass in the left back and left side of the neck in January. Three months before admission, the patient suffered a shrapnel injury in the neck and left the carotid artery at the evacuation center due to major bleeding. Suspected left vertebral artery traumatic aneurysm. Left vertebral arteriography showed a large aneurysm at the C2 level. The left middle cerebral artery is filled with an open posterior communicating artery. Angiographic findings revealed semicircular masses and prompted occlusion of the left carotid artery. The vertebral artery was reconstructed with autologous vein grafts. Make a left posterior midline incision at more than 1/3 of the neck and separate the distal part of the vertebral artery.
