目的探讨临床检测NHL骨髓侵犯的意义。方法应用流式细胞术和细胞形态学方法,同步分析34例NHL骨髓侵犯程度。结果①流式细胞术检测NHL患者骨髓细胞DNA异倍体率为618%(21/34),形态学分析BMI(+)率为177%(6/34),二法联检阳性率可达647%;②随患者临床分期增高,BMI(+)率也逐渐增加;BMI(+)者的S期细胞比率显著高于BMI(-)者(P<001);③化疗后,NHL患者骨髓细胞细胞凋亡水平明显升高(P<001),DNA异倍体率和SPF均有所降低。结论流式细胞术和形态学同步分析可提高NHL患者骨髓侵犯的阳性检测率,以评价肿瘤恶性度和化疗疗效;肿瘤化疗的重要机制之一,是药物诱导细胞凋亡。
Objective To explore the significance of clinical detection of bone marrow involvement in NHL. Methods Flow cytometry and cell morphology were used to analyze the extent of bone marrow invasion in 34 patients with NHL. Results 1 Flow cytometry detected the aneuploidy rate of bone marrow cells in 61 patients with NHL was 61.8% (21/34), and the morphological analysis BMI (+) rate was 17.7% (6/34). The rate can reach 647%; 2 With the increase of clinical staging of patients, the BMI (+) rate also gradually increases; the ratio of S-phase cells of BMI (+) is significantly higher than that of BMI (-) (P<001); 3 After chemotherapy, the apoptosis of myeloid cells in NHL patients was significantly increased (P<001), and the DNA heteroploid rate and SPF were all decreased. Conclusion Flow cytometry and morphological synchronization analysis can increase the positive rate of bone marrow invasion in patients with NHL to evaluate the degree of malignancy and efficacy of chemotherapy. One of the important mechanisms of cancer chemotherapy is drug-induced apoptosis.
