目的:调查浙江地区人群HPA系统遗传多态性特点,探讨临床血小板输注中HPA配型输注的应用效果。方法:采用PCR-SSP方法对400例献血员及54例血小板抗体筛检阳性患者进行血小板抗原HPA1-6,15系统基因分型,对54例患者进行分组血小板输注治疗效果分析。结果:血小板抗原HPA1-6,15系统中HPA-3的b基因频率0.51,系统b基因最高,HPA-15的杂合度0.33,系统杂合度最高,其余系统均以aa纯合子基因分型占绝对优势,超过90%。血小板HPA配型输注组CCI、PPR均明显优于ABO随机输注组。结论:在血小板临床输注治疗中应用HPA配型血小板可防止输注无效的发生率,从而有效提高血小板输注疗效和安全性,值得推广和应用。
Objective: To investigate the characteristics of genetic polymorphism of HPA in Zhejiang population and to explore the application effect of HPA with systemic infusion in clinical platelet transfusion. Methods: PCR-SSP method was used to genotype platelet antigen HPA1-6,15 in 400 blood donors and 54 patients with positive platelet antibodies, and 54 patients were treated with platelet transfusion. Results: The b gene frequency of HPA-3 was 0.51, the highest b gene of HPA-3 was HPA-1, the heterozygosity of HPA-15 was 0.33, and the highest heterozygosity of HPA-3 was found in other systems. Advantages, more than 90%. The CCI and PPR of platelet HPA matching infusion group were significantly better than that of ABO random infusion group. Conclusion: The application of platelet HPA in the clinical treatment of platelet transfusion can prevent the incidence of ineffective infusion, so as to effectively improve the efficacy and safety of platelet transfusion, which deserves promotion and application.
