目的观察分析自制半硬性双套管持续冲洗负压吸引法治疗结直肠癌术后吻合口瘘的临床效果。方法选取我院2015年2月—2016年2月收治的40例结直肠癌术后吻合口瘘患者,运用双色球随机分组法分为观察组(n=20)和对照组(n=20),对照组患者给予单腔引流管,观察组患者采用自制半硬性双套管持续冲洗负压吸引法治疗,分析两组患者治疗情况。结果在降钙素原降低时间、C反应蛋白降低时间、白蛋白上升时间、拔管时间、住院时间等方面,观察组患者较对照组患者明显缩短(P<0.05);观察组患者吻合口狭窄发生率5.00%,低于对照组的30.00%,观察组患者再手术率5.00%,低于对照组的20.00%,差异具有统计学意义(P<0.05)。结论针对结直肠癌术后吻合口瘘患者,采用自制半硬性双套管持续冲洗负压吸引法治疗,可改善全身营养状况,加快康复速度,缩短住院时间,值得临床积极推广和应用。
Objective To observe and analyze the clinical effect of self-made semi-rigid double cannula continuous flushing negative pressure suction method in treating postoperative anastomotic leakage of colorectal cancer. Methods Forty patients with anastomotic leakage of colorectal cancer after operation from February 2015 to February 2016 in our hospital were divided into observation group (n = 20) and control group (n = 20) by bicolor ball randomization method. Patients in the control group were given a single-lumen drainage tube. The patients in the observation group were treated with self-made semi-rigid double-cannula under continuous negative pressure suction therapy, and the treatment of the two groups was analyzed. Results Compared with the control group, the patients in the observation group were significantly shorter in the procalcitonin reduction time, C-reactive protein reduction time, albumin rise time, extubation time and hospital stay (P <0.05); in the observation group, the anastomotic stenosis The incidence was 5.00%, which was lower than 30.00% of the control group. The reoperation rate in the observation group was 5.00%, which was lower than that in the control group (20.00%). The difference was statistically significant (P <0.05). Conclusion For patients with colorectal cancer after anastomotic leakage, the self-made semi-rigid double-tube continuous negative pressure suction method can improve the nutritional status of the whole body, speed up the rehabilitation and shorten the length of hospital stay, which is worthy of clinical promotion and application.
