目的探讨“双管法”一体式倒Y型支架置入术治疗气管复合狭窄的可行性和疗效。资料与方法在透视下对2例气道复合狭窄的患者先经鼻腔置入通气导管,再经口-咽-气管释放气道支架。结果 2例患者支架置入均一次成功,在气管支架置入过程中,未出现出血、窒息。术后患者呼吸困难明显改善,气促评级由Ⅳ级改善为0级或Ⅰ级,脉氧饱和度(SPO2)由高流量吸氧时的70%、75%分别提高至自然呼吸时的90%、95%。结论在一体式倒Y型气管支架置入过程中,预先留置通气导管以维持呼吸和供氧,缓解了患者由于插入支架输送器引起和加剧的呼吸困难,能够更好的配合,使得气管支架放置更加安全,其技术可行,操作简单,值得临床推广应用。
Objective To investigate the feasibility and efficacy of “double-tube method ” integrated inverted Y-stent placement for tracheal compound stenosis. Materials and Methods 2 patients with airway stenosis under nasal ventilation were placed in the ventilation catheter, and then the airway-pharyngeal-airway stent was released. Results Both patients were successfully treated with stent implantation. No bleeding and asphyxia occurred during implantation of tracheal stent. Postoperative dyspnea was significantly improved, with improvement of grade Ⅳ to grade 0 or grade Ⅳ, and splenic oxygen saturation (SPO2) increased from 70% and 75% of high-flow oxygen inhalation to 90% , 95%. Conclusion In the integration of inverted Y-type tracheal stent placement process, the pre-placement ventilation catheter to maintain breathing and oxygen supply, ease the patient due to insertion of the stent conveyor caused by and aggravate dyspnea, better coordination, so that tracheal stent placement Safer, its technology is feasible, simple operation, it is worth clinical promotion and application.
