目的:探讨育龄期肥胖型多囊卵巢综合征(PCOS)妇女的内分泌激素及代谢特点。方法:选取2017年3月至2019年5月在湖南省中医药研究院附属医院的199例育龄期PCOS患者,根据体重指数(BMI)分为肥胖组91例和非肥胖组108例,所有患者均行内分泌激素检测、口服葡萄糖耐量试验(OGTT)及胰岛素释放试验(IRT),分析两组PCOS患者内分泌代谢指标。结果:肥胖型PCOS患者中胰岛素抵抗(IR)、高胰岛素血症(HI)、糖调节受损(IGR)、糖耐量减低(IGT)发生率明显升高(n P<0.05)。肥胖组PCOS患者促黄体生成素(LH)、促黄体生成素/促卵泡生成素(LH/FSH)比值、PRL低于非肥胖组(n P<0.05)。肥胖组PCOS患者空腹、餐后30、60、120、180 min胰岛素、餐后60、120 min血糖、血糖曲线下面积(AUCG)、胰岛素曲线下面积(AUCI)、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)均明显高于非肥胖组;肥胖组定量胰岛素敏感性指数(QUICKI)、李光伟指数均低于非肥胖组(n P<0.05)。n 结论:肥胖可能加剧PCOS患者体内的内分泌激素及代谢紊乱,建议检测葡萄糖耐量试验(OGTT)+胰岛素释放试验(IRT),及早诊断,及时干预。“,”Objective:To explore the characteristics of endocrine and metabolic in obese women with polycystic ovary syndrome (PCOS) in reproductive age.Methods:199 PCOS patients of childbearing age in Hunan Academy of Traditional Chinese Medicine Affiliated Hospital from March 2017 to May 2019 were selected and divided into obesity group (91 cases) and non obesity group (108 cases) according to body mass index (BMI). All patients underwent endocrine hormone detection, oral glucose tolerance test (OGTT) and insulin release test (IRT), and the endocrine and metabolic indexes of PCOS patients in the two groups were analyzed.Results:The incidence of insulin resistance (IR), hyperinsulinemia (HI), impaired glucose regulation (IGR) and impaired glucose tolerance (IRT) were significantly higher in obese PCOS patients (n P<0.05). The levels of luteinizing hormone (LH), luteinizing hormone/follicle stimulating hormone (LH/FSH) ratio and prolactin (PRL) in obese PCOS were lower than those in non obese PCOS (n P<0.05). The insulin, including fasting, 30 min, 60 min, 120 min, 180 min, postprandial blood glucose in 60 min, 120 min, areas under curves of glucose (AUCG), area under curve of insulin (AUCI), homeostatic model assessment of insulin resistance (HOMA-IR), homeostatic model assessment β cell function (HOMA-β) in obese group were significantly higher than those in non obese group (n P<0.05). The quantitative insulin sensitivity index (QUICKI) and Li Guangwei index of obesity group were lower than those of non obesity group(n P<0.05).n Conclusions:Obesity may aggravate the disorder of endocrine hormones and metabolism in PCOS patients. It is suggested that OGTT+ IRT should be detected for early diagnosis and timely intervention.
