目的比较睡前皮下注射长效甘精胰岛素与睡前皮下注射低精蛋白胰岛素(万苏林)分别联合口服阿卡波糖治疗2型糖尿病的降血糖效果和低血糖事件发生率。方法将70例初诊的2型糖尿病患者随机分为2组,每组各35例。甘精胰岛素组应用甘精胰岛素每晚睡前皮下注射联合阿卡波糖片每日3次口服;万苏林治疗组应用万苏林每晚睡前皮下注射联合阿卡波糖片每日3次口服。比较治疗后2组空腹血糖(FBG)、餐后2h血糖(P2hBG)、血糖达标时间、低血糖事件发生率、每日所用甘精胰岛素的剂量和万苏林的剂量及糖化血红蛋白(HbA1c)水平。结果甘精胰岛素组P2hBG、血糖达标时间、每日应用胰岛素的剂量均低于万苏林组,差异有统计学意义(P均<0.05);甘精胰岛素组有2例(5.7%)发生低血糖,万苏林组有10例(28.6%)发生低血糖,2组低血糖发生率比较差异有统计学意义(P<0.05);2组治疗后FBG、HbA1c水平差异无统计学意义(P均>0.05)。结论糖尿病患者应用甘精胰岛素联合口服阿卡波糖治疗2型糖尿病效果更好,血糖达标时间短,低血糖事件发生率低,患者受益更多。
Objective To compare the hypoglycemic effect and incidence of hypoglycaemia in patients with type 2 diabetes mellitus treated with subcutaneous injection of long-acting insulin glargine before bedtime and hypodermic insulin (Wan Su-lin) subcutaneously before bedtime before oral acarbose treatment. Methods Seventy two newly diagnosed type 2 diabetic patients were randomly divided into two groups, 35 cases in each group. Ganshu insulin group with glargine every night before bedtime subcutaneous injection of acarbose 3 times a day orally; Wan Surin treatment group Wan every night before bedrin subcutaneous injection combined with acarbose daily 3 Oral oral. The fasting blood glucose (FBG), 2h postprandial blood glucose (P2hBG), the time of reaching blood glucose, the incidence of hypoglycaemia, the dosage of insulin glargine daily and the dosage of Vansuarin and the levels of HbA1c . Results The dosages of P2hBG, target time of blood glucose and daily insulin dosage in glargine group were lower than those in wansulin group (all P <0.05), while those in insulin glargine group were low (5.7%) Hypoglycemia was observed in 10 cases (28.6%) in the blood sugar and Wan Suolin groups, and the incidence of hypoglycemia in the two groups was statistically significant (P <0.05). There was no significant difference in the FBG and HbA1c levels between the two groups (P All> 0.05). Conclusions The application of insulin glargine and acarbose orally to patients with type 2 diabetes is better, the time of blood glucose compliance is shorter, the incidence of hypoglycemia is lower, and patients benefit more.
