目的:分析38例全脑血管造影术的临床并发症种类、表现特点及处理措施。方法:回顾2013年9月-2015年9月期间我院为患者实施全脑血管造影术后,共38例发生并发症,分析并发症的具体情况及处理措施。结果:全脑血管造影术后38例患者出现并发症。其中全身并发症有8例(5例对比剂过敏、3例对比剂损伤肾功能);21例局部发生并发症(8例穿刺部位渗血、5例皮下血肿、4例血管口狭窄、4例导管打折);6例发生神经系统并发症(4例血管痉挛、2例血管栓塞);另有3例其他并发症(如心慌、恶心呕吐、低血压等)。临床对于穿刺部位渗血及皮下血肿情况未作任何特殊处理,因其可自行吸收;对于出现血管痉挛反射、栓塞的患者及时给予补液和药物治疗(如阿托品、多巴胺),有效处理后其症状明显改善;对比剂过敏的患者给予抗过敏药和地塞米松进行处理,缓解过敏症状;为肾功能损伤的患者采取护肾治疗,有效治愈。38例全脑血管造影术后并发症患者经临床处理后,均未出现任何后遗症。结论:由于全脑血管造影检查术是一种有创型检查,可导致相关并发症的发生,因此,术前、术中要充分做好并发症的防治准备,严格遵守操作规范,提高实施的安全性,进一步降低并发症发生率,有助于提高临床检查的效果。
OBJECTIVE: To analyze the types, performance characteristics and treatment measures of clinical complications in 38 cases of cerebral angiography. Methods: From September 2013 to September 2015, we retrospectively analyzed the complications of 38 patients who undergone total cerebral angiography in our hospital from January 2013 to September 2015. The complications and treatment measures were analyzed. Results: Complications occurred in 38 patients after total cerebral angiography. There were 8 cases of systemic complications (5 cases of contrast agent allergy, 3 cases of contrast agent damage renal function); 21 cases of local complications (bleeding in 8 puncture sites, 5 cases of subcutaneous hematoma, 4 cases of vascular stenosis, 4 cases 6 cases of neurological complications (4 cases of vascular spasm, 2 cases of vascular embolism); and 3 cases of other complications (such as palpitation, nausea, vomiting, hypotension, etc.). Clinic for the puncture site bleeding and subcutaneous hematoma situation without any special treatment, because they can absorb; for vasospasm reflex, embolism patients timely rehydration and drug treatment (such as atropine, dopamine), the effective treatment of the obvious symptoms Improve; Contrast allergy patients given anti-allergy drugs and dexamethasone for treatment, alleviating allergy symptoms; kidney function for patients with kidney protection, effective cure. Thirty-eight patients with post-cerebrovascular complications had no sequelae after clinical treatment. CONCLUSIONS: Whole-cerebrovascular angiography is an invasive procedure that can lead to the development of complications. Therefore, preoperative and postoperative preparations should be made to prevent and control complications, to strictly abide by the rules of operation and to improve the implementation of Safety, to further reduce the incidence of complications, help to improve the effectiveness of clinical examination.
