目的探索早孕期胼胝体发育不良(ACC)胎儿和前脑无叶无裂畸形(HPE)胎儿颅脑结构是否有改变。方法选择2013年06月~2013年10月在我院超声科检查的正常胎儿早孕期NT图像,及2011年1月1日~2013年10月31日我院诊断为ACC或HPE并且在我院超声科存有早孕期资料的胎儿。分别测量不同组早孕期(11~13+6周)胎儿颅脑正中矢状切面MD、FD值,计算MD/FD比值。所有数据采用SPSS 20.0软件包进行统计学处理。结果共有620例正常胎儿,5例ACC胎儿,13例HPE胎儿纳入研究。ACC胎儿和HPE胎儿早孕期胎儿颅脑正中矢状切面MD值、FD值、MD/FD比值差异无统计学意义(P均>0.05)。ACC及HPE胎儿颅脑正中矢状切面MD值、FD值、MD/FD比值均较正常胎儿发生明显改变(P<0.05)。所有ACC和HPE胎儿颅脑正中矢状切面MD值大于正常组胎儿,80%ACC及84.6%HPE胎儿颅脑正中矢状切面FD值小于正常组胎儿,ACC组及HPE组胎儿的MD/FD比值均大于1,而正常组MD/FD比值均小于1。结论早孕期ACC胎儿颅脑正中矢状切面MD、FD值,MD/FD比值较正常均发生明显改变,且这一改变在合并有ACC胎儿中也同样明显。因此早孕期胎儿颅脑正中矢状切面MD值、FD值、MD/FD比值可能是一个非常有潜力的早期发现ACC异常胎儿的新指标,为早期筛查ACC高危人群、发现不易察觉的神经系统或神经系统外畸形提供线索。
Objective To explore whether there is any change in the brain structure of the fetus with fetal corpus callosum (ACC) and the non-lobectomy (HPE) fetuses in the early pregnancy. Methods From January 2013 to October 2013 in our hospital ultrasound examination of normal fetuses in early pregnancy NT images, and January 1, 2011 ~ October 31, 2013 in our hospital diagnosed as ACC or HPE and in our hospital Department of ultrasound has information on the fetus of early pregnancy. The MD and FD values of median sagittal section of fetus in different groups of early pregnancy (11-13 + 6 weeks) were measured respectively, and the MD / FD ratio was calculated. All data were statistically analyzed using SPSS 20.0 software package. Results A total of 620 normal fetuses, 5 ACC fetuses and 13 HPE fetuses were included in the study. There were no significant differences in MD value, FD value and MD / FD ratio between fetus fetus and HPE fetus in early pregnancy fetus (all P> 0.05). The median sagittal MD, FD value and MD / FD ratio of ACC and HPE fetuses were significantly changed than those of normal fetuses (P <0.05). The MD value of median cerebral sagittal section of all ACC and HPE fetuses was larger than that of normal fetuses. The FD value of midsagittal plane of 80% ACC and 84.6% HPE fetuses was smaller than that of normal fetuses, ACC and HPE fetuses Were greater than 1, while the normal group MD / FD ratio were less than 1. Conclusion The median sagittal MD, FD value and MD / FD ratio of ACC fetuses in early pregnancy were significantly changed compared with the normal ones, and the change was also obvious in ACC fetuses. Therefore, the median sagittal MD, FD value and MD / FD ratio of fetus in early pregnancy may be a new potential indicator of fetal abnormalities in early detection of ACC.
