目的探讨自动乳腺全容积成像(ABVS)与声辐射力脉冲成像(ARFI)对乳腺病灶的诊断价值。方法在常规超声检查的基础上应用ABVS及ARFI技术对393例470个乳腺病变患者进行检查,以病理结果为金标准构建ROC曲线,比较其诊断价值。结果常规超声、ABVS及ARFI诊断乳腺癌的灵敏度、特异度、准确率分别为75.7%、88.7%、86.8%和80%、96.2%、93.8%和80%、91.2%、89.6%,曲线下面积(AUC)分别为0.817、0.886、0.882;ABVS联合ARFI诊断乳腺癌的灵敏度、特异度、准确率为91.4%、96.0%、95.5%,AUC为0.951。结论 ABVS及ARFI技术均有较高的诊断价值,两种方法相结合有助于乳腺病灶良恶性鉴别诊断。
Objective To investigate the diagnostic value of automatic breast total volume imaging (ABVS) and acoustic radiation pulse imaging (ARFI) in the diagnosis of breast lesions. Methods Based on routine ultrasound examination, 393 cases of 470 breast lesions were examined by ABVS and ARFI. The ROC curve was constructed according to the pathological results and the diagnostic value was compared. Results The sensitivity, specificity and accuracy of conventional ultrasound, ABVS and ARFI in the diagnosis of breast cancer were 75.7%, 88.7%, 86.8% and 80%, 96.2%, 93.8% and 80%, 91.2% and 89.6% (AUC) were 0.817,0.886,0.882 respectively. The sensitivity, specificity and accuracy of ABVS combined with ARFI in the diagnosis of breast cancer were 91.4%, 96.0% and 95.5%, respectively. The AUC was 0.951. Conclusion Both ABVS and ARFI techniques have high diagnostic value. The combination of the two methods is helpful for the differential diagnosis of benign and malignant breast lesions.
