目的探讨前列地尔联合低分子肝素钙治疗高脂血症性胰腺炎的临床疗效。方法选取盐边县人民医院2012年5月—2016年10月收治的高脂血症性胰腺炎患者148例,随机分为对照组和研究组,各74例。患者入院后均给予常规基础治疗,对照组患者给予前列地尔治疗,研究组患者在对照组基础上给予低分子肝素钙治疗,两组患者均持续治疗15 d。比较两组患者的临床疗效,胰腺炎症状明显改善时间、血清淀粉酶恢复正常时间及总禁食时间及治疗前后胰腺炎CT评分、三酰甘油(TG)水平。结果研究组患者临床疗效优于对照组(P<0.05)。研究组患者胰腺炎症状明显改善时间、血清淀粉酶恢复正常时间及总禁食时间短于对照组(P<0.05)。治疗前,两组患者胰腺炎CT评分及TG水平比较,差异无统计学意义(P>0.05);治疗后,研究组患者胰腺炎CT评分及TG水平低于对照组(P<0.05)。结论前列地尔联合低分子肝素钙治疗高脂血症性胰腺炎的临床疗效确切,可快速改善患者临床症状及体征,促进胰腺功能的恢复。
Objective To investigate the clinical efficacy of alprostadil combined with low molecular weight heparin in the treatment of hyperlipidemic pancreatitis. Methods 148 patients with hyperlipidemic pancreatitis treated in Yanbian People’s Hospital from May 2012 to October 2016 were randomly divided into control group and study group, with 74 cases in each group. All patients were given routine basic treatment after admission. Patients in the control group were treated with alprostadil. Patients in the study group were given low molecular weight heparin calcium on the basis of the control group. Both groups were treated for 15 days. The clinical efficacy, the time of improvement of symptoms of pancreatitis, the time to normal serum amylase recovery, the total fasting time and CT scores of pancreatitis and triglyceride (TG) before and after treatment were compared between the two groups. Results The clinical efficacy of the study group was better than that of the control group (P <0.05). Study group patients with pancreatitis symptoms significantly improved time, serum amylase recovery time and total fasting time is shorter than the control group (P <0.05). Before treatment, there was no significant difference in CT score and TG level between the two groups (P> 0.05). After treatment, CT score and TG level of pancreatitis in the study group were lower than those in the control group (P <0.05). Conclusion Alprostadil combined with low molecular weight heparin calcium treatment of hyperlipidemic pancreatitis clinical curative effect, can quickly improve the clinical symptoms and signs of patients and promote the recovery of pancreatic function.
