目的:分析房性期前收缩(PAC)前PR段和ST段压低患者的心电图特征。方法:选取PAC无PR段和ST段压低的40例住院患者为对照组,并选取同期PAC前PR段和ST段压低40例患者为观察组,分析两组患者的临床特点以及心电图特征。结果:两组患者的心、肺、脑器质性疾病发病率、平均年龄相比,差异具有统计学意义(P<0.05),观察组患者的提前指数(PI)、联律间期(CI)、P波时间、P波电压与对照组相比,差异具有统计学意义(P<0.05),但两组患者的PR间期比较则无明显变化,差异均无统计学意义(P>0.05)。观察组患者窦性心率明显高于对照组(P<0.05),观察组P波高大或切迹、P波增宽、P波时间、P波电压均明显优于对照组(P<0.05);两组患者QTc间期、PR间期比较,差异无统计学意义(P>0.05)。结论:PR段和ST段压低发生主要是因心房肌细胞生物电不稳定而导致,一旦发现该类型的PAC,可判定为高危病症,甚至合并严重的房性心律失常。
OBJECTIVE: To analyze the electrocardiographic features of patients with pre -trial contraction (PAC) before PR and ST depression. Methods: Forty inpatients with PAC without PR and ST depression were selected as the control group. Forty patients with PR and ST depression during the same period were selected as the observation group. The clinical features and ECG characteristics of the two groups were analyzed. Results: There were significant differences in the incidence and the average age of the organic, mental, and organic disease between the two groups (P <0.05). The PI, the inter-systolic interval (CI) ), P wave time, P wave voltage compared with the control group, the difference was statistically significant (P <0.05), but there was no significant difference between the two groups in the PR interval, the difference was not statistically significant (P> 0.05 ). The sinus rhythm of the observation group was significantly higher than that of the control group (P <0.05). In the observation group, the P wave height or notch, P wave broadening, P wave time and P wave voltage were significantly better than those in the control group (P <0.05). There was no significant difference in QTc interval and PR interval between the two groups (P> 0.05). CONCLUSIONS: The depression of PR and ST segments is mainly caused by the instability of bioelectrical activity of atrial myocytes. Once this type of PAC is found, it can be judged as a high-risk condition and even with severe atrial arrhythmia.
