1994年底,在江苏省3个肾综合征出血热高发地区,分别进行了国产沙鼠肾Ⅰ型苗和地鼠肾Ⅱ型苗的大规模人群接种;一年后,用同一厂家同一型别的疫苗进行了一针次加强接种,并进行了四年期的免疫持久性和防病效果观察,结果显示:沙鼠肾Ⅰ型苗的中和抗体在加强后抗体阳性率最高,随后逐年下降,至免后4年(加强后3年),其中和抗体阳性率仍高于免后1年;地鼠肾Ⅱ型苗的中和抗体在基础三针免后,阳转率最高,一年后经一针次加强,阳性率大大提高,至免后4年(加强后3年),其阳性率仍高于免后1年。对各个不同时期两种型别疫苗的同期进行比较,Ⅰ型苗的免后荧光抗体阳性率高于Ⅱ型苗,而中和抗体阳性率,则为Ⅱ型苗高于Ⅰ型苗。2种型别的疫苗在四年观察期间,接种人群共发生4例病人,有3例发生于免后1年内,此4例病人的血清学型别均不与所接种疫苗的型别相同,这些结果提示,免后1年加强接种是必要的,两种疫苗均有较好的防病效果,尤其对同型病例的保护率达100%,二种疫苗的保护期至少4年。另外,通过对全程接种者免后3年流行高峰前后的荧光抗体波动,和对接种与对照人群发生的病例的病情进行比较研究,显示,国产两种型别的HFRS灭活疫苗免后无免疫效果(感染)增强反应。
