目的 评价光动力疗法 (PDT)联合局部化疗治疗食管贲门癌的疗效。方法 应用PDT治疗食管贲门癌 110例 ,并对其中 70例联合应用内镜下局部注射化疗药物。对每一患者均先静脉滴注癌光啉 (PSD - 0 0 7) 3～ 5mg/kg ,于用药后 2 4、48或 72h分别以激光照射肿瘤局部。PDT联合局部化疗组除PDT治疗外 ,于每次光照前肿瘤局部注射 5 -FU 2 5 0～ 5 0 0mg或平阳霉素 8mg。结果 PDT联合局部化疗组的近期显效率高于单纯PDT组 (P<0 .0 5 )。部分病例随访 10～ 18个月 ,PDT联合局部化疗组的平均生存期较单纯PDT组延长 (P <0 .0 1)。结论PDT联合局部化疗治疗食管贲门癌安全而有效 ,并能提高PDT的治癌效果
Objective To evaluate the efficacy of photodynamic therapy (PDT) combined with local chemotherapy in the treatment of esophageal and cardiac cancer. Methods 110 patients with esophageal and cardiac cancer were treated with PDT, and 70 of them were combined with endoscopic chemotherapy. For each patient, an intravenous instillation of cancer photomorpholine (PSD-07) 3 to 5 mg/kg was first performed. The tumor was irradiated with laser light at 24, 48, or 72 hours after administration. In the PDT combined with local chemotherapy group, in addition to PDT treatment, the tumor was injected with 5-FU 250-500 mg or Pingyangmycin 8 mg before each irradiation. Results The effective rate of PDT combined with local chemotherapy was higher than that of PDT alone (P<0.05). Some patients were followed up for 10 to 18 months. The mean survival time of PDT combined with local chemotherapy group was longer than that of PDT alone (P < 0.01). Conclusion PDT combined with local chemotherapy is safe and effective in the treatment of esophageal and cardiac cancer, and can improve the therapeutic effect of PDT.
