目的:分析中药处方中存在的不规范现象。方法:随机选取医院2014年10月~2015年10月开具的中药处方中的600张,调查中药处方的规范情况,统计其中存在的不规范现象,分析不规范存在的原因,提出规范中药处方的对策。结果:经过统计结果可知,在600张中药处方中,存在不规范现象处方78张,占13.0%。在78张不规范处方中,处方前记不全4例,处方书写字迹潦草10例,中药药名书写不规范14例,漏脚注4例,中药物重复10例,处方中涂改处缺乏医生签名4例,用法不明9例,剂量不规范3例,药材品种不明确7例,药品配伍不当8例,同种药物名称混淆5例。结论:中药处方不规范现象比较多,医院应加强对中药处方规范重要性的认识,通过规范划管理,保证中药处方规范,提高用药的安全性。
Objective: To analyze the nonstandard phenomenon existing in traditional Chinese medicine prescriptions. Methods: 600 hospital prescriptions were randomly selected from traditional Chinese medicine prescriptions issued by the hospital from October 2014 to October 2015 to investigate the prescriptions of traditional Chinese medicine (TCM) prescriptions and statistics of the non-standardization among them. The reasons for the non-standardization were analyzed, Countermeasures. Results: The statistical results show that in the 600 prescriptions of Chinese medicine, there are 78 non-standard prescriptions, accounting for 13.0%. Among the 78 non-standard prescriptions, 4 cases were premedicated in prescriptions, 10 were illegible in prescription writing, 14 were not standardized in prescriptions of Chinese medicine, 4 were missed footprints, 10 were repeated in Chinese medicines, and there was a lack of physician’s signature 4 Cases, 9 cases of unknown use, 3 cases of non-standard dose, drug species is not clear in 7 cases, improper compatibility of drugs in 8 cases, the same drug name confusion in 5 cases. Conclusion: There are many non-standard prescriptions for traditional Chinese medicine. The hospital should strengthen its understanding of the importance of traditional Chinese medicine prescriptions, and ensure the safety of medication by standardizing management, ensuring prescriptions of traditional Chinese medicines.
