目的:观察高压氧(HBO)对丁胺卡那霉素(AMK)药代动力学的影响。方法:对8名健康成人单剂量肌注AMK5mg/kg,分别在以下3种状态下对其药代动力学进行了研究:正常状态,1次0.25MPa(2.5ATA)HBO和10次0.25MPaHBO。用荧光偏振免疫法测定AMK的血药浓度。结果:经配对t检验表明,连续10次0.25MPaHBO治疗后可显著减小AMK的消除半衰期(P<0.05),而对其它药代动力学参数均无显著影响(P>0.05)。结论:HBO治疗可能通过增加尿量,使AMK经肾脏排泄增多,因而显著减小其消除半衰期。在临床上HBO与AMK合用时,最好监测AMK的血药浓度,适时调整给药剂量并检查肾功能和听力,防止不良后果的发生。
Objective: To observe the effects of hyperbaric oxygen (HBO) on the pharmacokinetics of amikacin (AMK). Methods: Eight adult healthy single dose AMK5mg / kg, respectively, in the following three states its pharmacokinetics were studied: a normal condition, a 0.25MPa (2.5ATA) HBO and 10 times 0 .25MPaHBO. Fluorescence polarization immunoassay was used to determine the plasma concentration of AMK. Results: The paired t-test showed that the elimination half-life of AMK was significantly reduced after 10 consecutive 0.25MPa HBO treatments (P <0.05), but no significant effect on other pharmacokinetic parameters (P> 0.05 ). Conclusions: HBO treatment may significantly reduce its elimination half-life by increasing urine output and excreting AMK through the kidneys. In clinical HBO and AMK combined, it is best to monitor the plasma concentration of AMK, timely adjustment of dose and check renal function and hearing, to prevent the occurrence of adverse consequences.
