小型猪上颌窦内提升穿破上颌窦粘膜同期植入种植体的骨结合情况的研究

目的观察上颌窦内提升术中窦底粘膜穿通对同期植入的种植体骨结合的影响,以及生物膜的应用效果。方法试验用小型猪3只,X线确定上颌窦底位置,全麻下拔除小型猪双侧上颌相应位置的牙,6个种植位点随机分为3组,实验A组穿破窦底同期植入种植体,实验B组穿破窦底粘膜加生物膜同期植入种植体,对照组窦底粘膜完整。术后6个月观察大体标本,做X线检查,做带种植体的骨磨片,观察种植体的骨结合情况及窦底的愈合情况。结果大体标本显示种植体无松动,A组种植体暴露在上颌窦内,B组与对照组窦底粘膜完整。X线显示3组的种植体周围的骨质均匀,没有阴影。骨磨片镜下观察骨结合情况,3组没有差别。结论上颌窦底粘膜的穿通对种植体骨结合没有影响,窦底粘膜穿通后加生物膜有利于窦底粘膜的愈合。 Objective To observe the effect of sinus mucosa penetration on the osseointegration of implants implanted in maxillary sinus and the application of biofilm. Methods Three miniature pigs were examined by X-ray. The maxillary sinus floor was determined by X-ray. The teeth of bilateral maxilla were removed under general anesthesia. The six planting sites were randomly divided into three groups. In group A, Into the implant, experimental group B puncture sinus mucosa plus biofilm implantation implant simultaneously, the control group sinus mucosa intact. The gross specimens were observed 6 months after the operation, and X-ray examination was performed. The bone-worn pieces with the implants were observed and the bone union of the implants and the healing of the sinus floor were observed. Results The gross specimen showed no loosening of implants. The implants in group A were exposed in the maxillary sinus and the sinus mucosa in group B and the control group were intact. X-ray showed that the bone around the implants in 3 groups was uniform and without shadow. Bone grinding microscope observation of bone union, there was no difference in the three groups. Conclusion Perforation of maxillary sinus mucosa has no effect on osseointegration of implants. The perfusion of the sinus mucosa with biofilm is helpful for the healing of sinus mucosa.
