目的:探讨磷脂酰肌醇蛋白聚糖3(GPC3)mRNA和甲胎蛋白(AFP)mRNA在肝细胞癌(HCC)患者外周血中联合检测对HCC诊断价值。方法:采用荧光定量RT-PCR技术检测72例HCC患者、22例肝良性病变组和20例健康组外周血中GPC3 mRNA和AFP mRNA表达。结果:HCC患者GPC3 mRNA和AFP mRNA表达显著高于肝良性病变组和健康组(P<0.05);肝良性病变组和健康组中表达无统计学意义(P>0.05)。两者表达与肿瘤大小、是否侵袭和病理分级有关(P<0.05);AFP mRNA表达还与血清AFP浓度有关(P<0.05);年龄、性别、有无HbsAg感染和包膜与两者表达无关(P>0.05)。单项检测灵敏度、准确度较低;联合检测灵敏度达90.3%,准确度达92.1%;两者在HCC中表达未存在直接关联(P>0.05)。结论:联合检测GPC3 mRNA和AFP mRNA的表达有助于HCC的早期临床诊断。
Objective: To investigate the diagnostic value of combined detection of glypican 3 (GPC3) mRNA and α-fetoprotein (AFP) mRNA in peripheral blood of patients with hepatocellular carcinoma (HCC). Methods: The expression of GPC3 mRNA and AFP mRNA in peripheral blood of 72 patients with HCC, 22 patients with benign liver disease and 20 healthy controls were detected by real-time fluorescence quantitative RT-PCR. Results: The expression of GPC3 mRNA and AFP mRNA in HCC patients was significantly higher than that in benign liver disease group and healthy group (P <0.05). There was no significant difference between benign liver disease group and healthy group (P> 0.05). The expression of AFP mRNA was correlated with the serum AFP concentration (P <0.05). The age, sex, presence or absence of HbsAg infection and the expression of the envelope were not related to the expression of AFP mRNA (P> 0.05). The single detection sensitivity and accuracy were lower. The combined detection sensitivity was 90.3% and the accuracy was 92.1%. There was no direct correlation between them in HCC (P> 0.05). Conclusion: The combined detection of GPC3 mRNA and AFP mRNA expression contributes to the early clinical diagnosis of HCC.
