目的探讨社区卫生服务中心与上级综合医院间的双向转诊机制对早期发现胃肠肿瘤的作用。方法选择2007—2012年因症就诊及60岁以上老年免费体检途径发现的胃癌及大肠癌人群作为研究对象,其中,2007—2009年设立双向转诊前发现的胃癌及大肠癌人群作为对照组,2010—2012年实行双向转诊后发现的胃癌及大肠癌人群作为转诊组。设计胃肠肿瘤双向转诊机制,通过采用非同期随机对照试验的方法,评价胃癌和大肠癌早发现双向转诊的效果。结果对照组胃癌、大肠癌早发现数分别为0、0例,转诊组本别为1、8例,均有所增加。结论有针对性的双向转诊机制对提高胃肠肿瘤早期发现率作用十分明显,值得推广。
Objective To explore the effect of bidirectional referral mechanism between community health service centers and superior general hospitals on the early detection of gastrointestinal tumors. Methods From 2007 to 2012, patients with gastric cancer and colorectal cancer who were diagnosed during the period from 2007 to 2009 and free medical examination over 60 years of age were selected as the research subjects. Among them, the patients with gastric cancer and colorectal cancer detected before 2007-2009 were established as control group, 2010-2012 two-way referral after the discovery of gastric cancer and colorectal cancer as a referral group. To design a bi-directional referral mechanism of gastrointestinal tumors and to evaluate the effect of bi-directional referral detected early in gastric cancer and colorectal cancer by using non-synchronous randomized controlled trials. Results In the control group, the number of early detection of gastric cancer and colorectal cancer were 0 and 0, respectively, while the number of referral group was 1 and 8, both of which increased. Conclusions Targeted bidirectional referral mechanism is very effective in improving the early detection rate of gastrointestinal tumors and is worth promoting.
