目的探讨对社区高血压患者应用全科团队式社区健康管理方案的临床价值。方法纳入社区卫生服务中心门诊的252例原发性高血压患者为研究对象:观察组126例接受全科团队式护理服务;对照组126例仅接受常规门诊治疗。持续6个月后,对比两组血压变化及健康行为模式。结果两组干预前血压无显著差异(P>0.05),干预后均有改善,观察组干预后舒张压(DBP)与收缩压(SBP)均明显低于对照组,差异有统计学意义(P<0.05);观察组患者干预后定期测量血压、坚持运动、合理饮食及遵医嘱用药等健康行为模式比率均显著高于对照组,差异有统计学意义(P<0.05)。结论对社区高血压患者,实施全科团队式社区健康管理,有助于改善其血压状况及健康行为状况,值得推广。
Objective To explore the clinical value of community-based community health management programs for community-based hypertensive patients. Methods A total of 252 patients with essential hypertension admitted to community health service center were included in the study: 126 cases in the observation group received team nursing service and 126 cases in the control group received routine outpatient treatment only. After 6 months, compared two groups of blood pressure changes and healthy behavior patterns. Results There was no significant difference in blood pressure between the two groups before intervention (P> 0.05), and after intervention there was an improvement. The DBP and SBP of the observation group were significantly lower than those of the control group (P <0.05). The ratio of healthy behavior patterns such as regular measurement of blood pressure, exercise, reasonable diet and medication prescribed by the observation group after intervention was significantly higher than that of the control group (P <0.05). Conclusions Community-based community-based community health management for hypertensive patients in community can help to improve their blood pressure status and health behaviors, which is worthy of promotion.
