据《中华医学杂志》1994年74卷第5期报道 上海医科大学儿科医院黄国英等,应用二维伪彩色多普勒超声心动图(CDE)检测了1000例单纯性室间隔缺损(IVSD),其中膜周部缺损854例,漏斗部缺损131例,肌部缺损15例。 结果经手术证实,CDE诊断IVSD的敏感性达100%,对缺损的定位准确率为98%,对缺损大小的估测与手术结果呈显著正相关(r=0.893,P<0.001)。与心导管测值比较,用连续波式多普勒技术估测的肺动脉收缩压与之具有良好相关性(r=0.838,P<0.001)。结合M型超声测量右心室腔内径,肺动脉与主动脉根部内径比值(MPA/AO)发现,MPA/AO>1.27及右心室增大常提示肺动脉高压。随访结果表明,IVSD自然闭合率为26.3%,自然闭合情况与缺损部位有关。
According to the “Chinese Medical Journal” Volume 74, Volume 5, 1994 reported Huang Guoying, Shanghai Medical University Pediatric Hospital, the application of two-dimensional pseudo-color Doppler echocardiography (CDE) detected 1000 cases of simple ventricular septal defect (IVSD) Peripheral membrane defect in 854 cases, funnel defect in 131 cases, muscle defect in 15 cases. Results The accuracy of CDE in the diagnosis of IVSD was 100%. The accuracy of positioning was 98%. The size of the defect was positively correlated with the surgical outcome (r = 0.893, P <0.001). Pulmonary artery systolic blood pressure assessed by continuous wave Doppler technique was associated with a good correlation with cardiac catheterization (r = 0.838, P <0.001). Combined with M-mode ultrasound to measure the diameter of the right ventricular cavity, the ratio of pulmonary artery to aortic root diameter (MPA / AO) found that MPA / AO> 1.27 and right ventricular enlargement often prompted pulmonary hypertension. The follow-up results showed that the natural closure rate of IVSD was 26.3%, and the natural occlusion was related to the defect site.
