目的:评价幼虫冰冻切片抗原间接荧光抗体试验(IFAT)诊断旋毛虫病的价值。方法:应用纯净幼虫冰冻切片间接荧光抗体试验检测感染旋毛虫小鼠和旋毛虫病患者血清中的抗旋毛虫抗体,在患者发病后及应用阿苯达唑治疗后不同时间采集血清,以其他寄生虫病患者和健康人血清为对照。结果:15只小鼠感染旋毛虫后2wk即可检出抗体,467例旋毛虫病患者的抗体阳性率为86.5%,而丝虫病、包虫病患者各45例和健康人血清56份均为阴性。48份并殖吸虫病患者的交叉反应率为16.67%。旋毛虫病患者发病后1wk的抗体阳性率为70.21%,至发病后2wk、3wk及4wk则分别升至91.0%、95.8%及100%。幼虫冰冻切片的抗原活性于-20℃可保存至5年。旋毛虫病患者治疗后1wk抗体阳性率从治疗前的87.5%上升到100%,血清抗体滴度比治疗前也有明显升高,治疗后1个月的抗体转阴率为24%,至治疗后4个月达75%。结论:幼虫冰冻切片间接荧光抗体试验诊断旋毛虫病具有较高的敏感性和特异性,并可用于疗效考核
OBJECTIVE: To evaluate the diagnostic value of indirect fluorescent antibody test (IFAT) for diagnosis of trichinosis in larval frozen sections. Methods: Serum anti-Trichinella antibodies in mice infected with Trichinella spiralis and Trichinellosis were detected by indirect fluorescent antibody assay of frozen section of pure larvae. Serum was collected at different time after onset and after treatment with albendazole, Insect disease and healthy human serum as a control. RESULTS: Antibodies were detected 2 weeks after infected with Trichinella in 15 mice. The positive rate of antibodies in 467 cases of Trichinella was 86.5%, while that of filariasis, echinococcosis and serum of healthy people were 56 All were negative. The cross-reactivity rate of 48 paragonimiasis patients was 16.67%. The positive rate of antibody at 1wk after onset was 70.21% in patients with Trichinella, and rose to 91.0%, 95.8% and 100% at 2wk, 3wk and 4wk respectively after onset. The antigenic activity of frozen section of larvae can be stored at -20 ℃ until 5 years. After 1wk treatment, the positive rate of antibody in patients with Trichinellosis increased from 87.5% before treatment to 100%, and the serum antibody titer was significantly higher than that before treatment. After one month of treatment, the antibody negative rate was 24% 75% after 4 months of treatment. Conclusion: Indirect fluorescent antibody test of frozen sections of larvae has high sensitivity and specificity for the diagnosis of Trichinellosis, and can be used for evaluation of curative effect
