目的探讨甲壳质酶蛋白(YKL-40)在支气管哮喘患者血清中的表达及意义。方法 42例支气管哮喘患者分为病情未控制组22例和基本控制组20例,18例健康正常成年人为对照组,采用ELISA法检测其血清中的YKL-40、血清总IgE水平,并记录血嗜酸粒细胞计数、FEV1%pred及ACT评分结果。结果未控制组患者血清YKL-40、血清总IgE、嗜酸粒细胞均较基本控制组和对照组升高明显,差异具有统计学意义(P<0.05)。基本控制组与对照组患者各项指标无明显差异性,未控制组患者血清YKL-40升高与血清总IgE的表达无相关性,与血嗜酸粒细胞计数呈正相关,与FEV1%pred、ACT评分呈负相关。结论 YKL-40水平在未控制组哮喘患者血清中升高明显,且与嗜酸性粒细胞升高呈正相关,与肺功能呈负相关,可作为哮喘病情控制程度的一项血清学指标。
Objective To investigate the expression and significance of chitinase (YKL-40) in the serum of patients with bronchial asthma. Methods Twenty-four patients with bronchial asthma were divided into two groups: uncontrolled group (n = 22) and basic control group (n = 20), and control group (n = 18). The serum levels of YKL-40 and total serum IgE were measured by ELISA. Eosinophil count, FEV1% pred and ACT scores. Results The serum levels of YKL-40, total serum IgE and eosinophils in control group were significantly higher than those in control group and control group (P <0.05). There was no significant difference between the basic control group and the control group. Serum YKL-40 was not correlated with the level of total serum IgE in control group, and positively correlated with blood eosinophil count, but not with FEV1% pred, ACT score was negatively correlated. Conclusion The level of YKL-40 is significantly elevated in the serum of uncontrolled asthma patients and positively correlated with the increase of eosinophils. It is negatively correlated with pulmonary function and may be used as a serological marker for the control of asthma.
