本文用高效液相色谱法检测慢性阻塞性肺疾病(COPD)合并肺内感染患者支气管肺泡洗出液(BALF)和血浆的头孢氨噻肟(CTX)的药物浓度及白蛋白(Alb)浓度。结果 BALF 的混液 CTX 浓度显著低于第1次灌洗液的浓度(P<0.05)、高于第2次灌洗液的浓度(P<0.05)。而第1次灌洗液 CTX浓度明显高于第2次灌洗液的浓度(P<0.01)。各液间的 Alb 浓度无明显差异。BALF 的 CTX 浓度与血药浓度呈正相关(P<0.01),与 Alb 浓度无相关(P>0.05)。说明 CTX 有较强的穿过支气管和肺泡-毛细血管膜屏障的能力,治疗肺感染症时维持一定的血药浓度是必要的。
In this paper, the drug concentrations and albumin (Alb) concentrations of cefotaxime (CTX) in bronchoalveolar lavage fluid (BALF) and plasma were determined by high performance liquid chromatography in patients with chronic obstructive pulmonary disease (COPD) and pulmonary infection. Results The concentration of CTX in BALF was significantly lower than that of the first lavage (P <0.05) and higher than that of the second lavage (P <0.05). The first lavage fluid CTX concentration was significantly higher than the second lavage fluid concentration (P <0.01). There was no significant difference in the concentration of Alb between each solution. The CTX concentration in BALF was positively correlated with plasma concentration (P <0.01), but not with Alb concentration (P> 0.05). CTX has a strong ability to cross the bronchial and alveolar-capillary membrane barrier, the treatment of lung infection to maintain a certain blood concentration is necessary.
