目的探讨代谢综合征(MS)对糖尿病(DM)合并无症状脑梗死(SCI)的影响。方法分析2006年1月-2009年6月入院的经头颅CT或核磁确诊DM合并SCI(A组)患者38例,非DM合并SCI(B组)36例,检测收缩压(SBP)、舒张压(DBP)、体质量指数(BMI)、腰围(WC),静脉采血测空腹血糖(FPG)、血清甘油三脂(TG)、血清髙密度脂蛋白胆固醇(HDL-C)的水平。结果 A组MS的发病率为34.2%明显高于B组19.4%(P<0.05)。A组SBP、FPG明显增高,HDL-C水平低于B组(P<0.05)。Logistic回归分析显示,糖尿病合并SCI发病密切相关的是高SBP和低LDL-C,OR值分别为2.11和1.15,95%CI为1.81~3.67、1.03~1.45(P<0.05)。结论 MS使DM合并SCI发病率增高,有效控制SBP和升高HDL-C对预防DM合并SCI有重要临床意义。
Objective To investigate the effect of metabolic syndrome (MS) on diabetic asymptomatic cerebral infarction (SCI). Methods Thirty-eight patients with DM combined with SCI (group A) and 36 patients without DM combined with SCI (group B) who were admitted to our hospital from January 2006 to June 2009 were analyzed by CT or MRI. SBP, DBP, WC, FPG, TG and HDL-C were measured. Results The incidence of MS in group A was significantly higher than that in group B (34.2% vs 19.4%, P <0.05). Group A SBP, FPG was significantly higher, HDL-C levels were lower than the B group (P <0.05). Logistic regression analysis showed that the association between diabetes and SCI was closely related to the high SBP and low LDL-C, with odds ratios of 2.11 and 1.15, 95% CI of 1.81-3.67 and 1.03-1.45, respectively (P <0.05). Conclusion MS makes DM combined with SCI increased incidence of effective control of SBP and HDL-C increase the prevention of DM with SCI has important clinical significance.
