人乳头瘤病毒DNA检测与液基薄层细胞学检查在宫颈疾病筛查中的应用

目的:探讨液基薄层细胞学与高危型HPV-DNA检测在妇科宫颈疾病筛查的应用及其临床意义。方法:分析733名妇女宫颈刷出物中13种高危型HPV-DNA检测结果,其中227例同时进行TCT细胞学检查。根据细胞学诊断结果分为4组,同时对各组HPV DNA检测结果进行统计学分析。结果:733名妇女高危型HPV感染率为16.2%。227例TCT结果显示各年龄组TCT阳性率差别没有统计学意义(P=0.203)。各分组的高危型HPV检出率分别为9.5%(17/179)、63%(17/27)、90.9%(10/11)和80%(8/10),差别具有统计学意义(P=0.000),在TCT阳性的病例中(A0以上),高危型HPV DNA检出率随着细胞异常级别的升高而升高的趋势并不明显。结论:联合应用高危型HPV检测和液基薄层细胞学(TCT)检查可以提高宫颈疾病筛查阳性率。 Objective: To investigate the application of liquid-based thin-layer cytology and high-risk HPV-DNA testing in gynecological cervical disease screening and its clinical significance. METHODS: Thirteen high-risk HPV-DNA tests were performed in 733 women with cervical brushes, of which 227 were TCT cytology at the same time. According to the results of cytology were divided into four groups, at the same time on each group of HPV DNA test results for statistical analysis. Results: The prevalence of high-risk HPV infection in 733 women was 16.2%. 227 cases of TCT results showed that the TCT positive rate of each age group difference was not statistically significant (P = 0.203). The detection rates of high-risk HPV in each sub-group were 9.5% (17/179), 63% (17/27), 90.9% (10/11) and 80% (8/10), respectively = 0.000). In the TCT-positive cases (above A0), the detection rate of high-risk HPV DNA tended to increase with the abnormal level of cells was not obvious. Conclusion: The combination of high-risk HPV testing and liquid-based TLCT can improve the screening rate of cervical disease.
