目的 了解皮肌炎合并恶性肿瘤的临床特点。方法 对 32例明确诊断为皮肌炎合并恶性肿瘤患者 ,从肌酸激酶、恶性肿瘤的发生时间、种类、治疗、预后等方面进行回顾性分析。结果 恶性肿瘤可与皮肌炎同时发生(13/ 32 ) ,亦可发生在其之前 (5 / 32 )或之后 (14/ 32 )。肿瘤可出现在任何部位 ,其中以肺、胃肠道、乳腺、卵巢多见。皮肌炎的治疗及预后与恶性肿瘤的控制情况密切相关。治疗上采用包括化疗在内的综合治疗措施更有利于控制皮肌炎的病情。结论 皮肌炎患者易合并各种恶性肿瘤 ,常预后不佳。对明确诊断为皮肌炎的患者有必要进行恶性肿瘤的筛选。
Objective To understand the clinical features of dermatomyositis complicated with malignant tumors. Methods 32 patients with definite diagnosis of dermatomyositis complicated with malignant tumors were retrospectively analyzed from the time, type, treatment and prognosis of creatine kinase and malignant tumors. Results Malignant tumors can occur concurrently with dermatomyositis (13/32). They can also occur before (5/32) or after (14/32). Tumors can occur in any part of the body, with the lung, gastrointestinal tract, breast, and ovary being more common. The treatment and prognosis of dermatomyositis are closely related to the control of malignant tumors. The use of comprehensive treatment including chemotherapy in treatment is more conducive to the control of dermatomyositis. Conclusion Patients with dermatomyositis are easily associated with various malignancies and often have poor prognosis. It is necessary to screen for malignant tumors in patients with a definite diagnosis of dermatomyositis.
