瘙痒是指一种不愉快的、并即刻引起搔抓欲望的感觉,最常见于皮肤病,也可能是潜在的系统性疾病的皮肤表现。皮肤T细胞淋巴瘤(cutaneous T-cell lymphomas,CTCL)是一种可引起瘙痒的威胁生命的皮肤肿瘤。瘙痒可以继发于CTCL,也可以是CTCL的原发表现。瘙痒通常在CTCL疾病进展期或某些亚型中,症状较为严重,如亲毛囊性CTCL、Sézary综合征(Sézary syndrome,SS)以及红皮病型蕈样肉芽肿(Mycosis fungoides,MF),一般抗瘙痒治疗难以抑制。目前,关于CTCL瘙痒机制的研究处于起始阶段,研究发现多种介质与其相关,如白介素31、P物质、蛋白酶、胃泌素释放肽和神经生长因子等,且其治疗也主要依赖治疗原发病,针对性的靶向治疗很少,控制瘙痒多依赖于经验用药。本文对CTCL相关性瘙痒研究进展进行综述并做文献复习。
