目的观察开心胶囊对犬心脏停搏复苏后心脏功能的影响。方法将15只健康杂种犬,随机分为假手术组、模型组和开心胶囊组,复制心脏骤停复苏模型。选择开胸前、除颤后即刻、自主循环恢复5,15,30,60,90,120,180min9个时间点记录各心功能指标。结果模型组与开心胶囊组主动脉平均压(MABP)、左心室作功指数(LVWI)、心肌耗氧量(D-P)、心率(HR)在复苏后的5min一过性增高,15min后MABP、LVWI、心输出量(CO)、心脏指数(CI)和冠脉灌注压(CPP)持续下降,30min后D-P、HR开始下降,与模型组相比,开心胶囊组下降较慢(P<0.05)。30min后开心胶囊组CO、CI和CPP与模型组相比仍维持在较高水平(P<0.05)。模型组和开心胶囊组MPAP、肺毛细血管楔压(PCWP)、总外周循环阻力(TSVR)与总肺循环阻力(TPVR)在复苏后5min明显上升(P<0.05),而开心胶囊组各指标上升较缓慢,在30min后与模型组相比有显著性差异(P<0.05)。复苏后模型组血液温度(T)下降较假手术组、开心胶囊组明显(P<0.05)。结论心脏骤停复苏后存在左室收缩功能障碍,导致MABP降低、PCWP增高等血流动力学指标的紊乱。开心胶囊在一定程度上能升高血压和增加心输出量,改善复苏后血液动力学,对心脏骤停复苏后心脏功能具有一定的恢复作用。
Objective To observe the effect of Kaixin capsule on cardiac function after cardioplegic resuscitation in dogs. Methods 15 healthy mongrel dogs were randomly divided into sham operation group, model group and Kaixin capsule group, and the cardiopulmonary arrest resuscitation model was duplicated. The cardiac function indexes were recorded at 9 time points before chest opening, immediately after defibrillation, and 5, 15, 30, 60, 90, 120, and 180 minutes of recovery of spontaneous circulation. Results The average aortic pressure (MABP), left ventricular work force index (LVWI), myocardial oxygen consumption (DP) and heart rate (HR) in the model group and Kaixin capsule group increased transiently after 5 minutes of resuscitation, and MABP, after 15 minutes. LVWI, cardiac output (CO), cardiac index (CI), and coronary perfusion pressure (CPP) continued to decline. After 30 minutes, DP and HR began to decline. Compared with the model group, Kaixin capsule group decreased slowly (P<0.05). . After 30 minutes, CO, CI and CPP in the Kaixin capsule group were still higher than those in the model group (P<0.05). MPAP, pulmonary capillary wedge pressure (PCWP), total peripheral vascular resistance (TSVR) and total pulmonary vascular resistance (TPVR) in model group and Kaixin capsule group increased significantly at 5 min after resuscitation (P<0.05), while those in Kaixin capsule group increased. It was slower and there was a significant difference (P<0.05) compared with the model group after 30 min. After the resuscitation, the blood temperature (T) decreased significantly in the model group compared with the sham-operated group and the Kaixin capsule group (P<0.05). Conclusion There is left ventricular systolic dysfunction after resuscitation from cardiac arrest, which leads to a decrease in MABP and an increase in PCWP. Kaixin Capsule can increase blood pressure and increase cardiac output to a certain extent, improve hemodynamics after resuscitation, and have a certain recovery effect on cardiac function after cardiac arrest.
