目的评价64层螺旋CT门静脉容积成像技术在门静脉高压症中的应用价值。方法 8例患者均行64层螺旋CT增强扫描,采用容积再现、最大强度密度投影、多平面重建等后处理技术进行门静脉血管系统重建成像,观察其病理状态下的血管影像学特征。结果 38例患者,经门静脉血管造影检查后均证实为门静脉高压症患者,门静脉系统及侧支循环的建立均得到良好的显示,显示血管分级可达4~5级,门静脉系统癌栓也得到良好的显示。结论 64层螺旋CT门静脉成像技术实现了任意角度、立体观察门静脉高压症的门静脉系统及侧支循环的血管情况,对于门静脉高压症诊断及为制定手术治疗方案具有重要指导意义。
Objective To evaluate the value of 64-slice spiral CT portography in portal hypertension. Methods Eight cases of patients underwent spiral CT enhancement scan. The reconstruction of the portal vein system was performed by means of volume rendering, maximum intensity density projection, multiplanar reconstruction and other postprocessing techniques. The vascular imaging features under the pathological conditions were observed. Results Thirty-eight patients confirmed portal hypertension after portal vein angiography. The establishment of portal vein system and collateral circulation were well demonstrated. The grade of vascular grafts reached 4 to 5 and the portal vein thrombosis was also good The display. Conclusion 64-slice spiral CT portal vein imaging technology can realize the observation of the portal vein system and collateral circulation vessels at any angle and in three dimensions. It has important guiding significance for the diagnosis of portal hypertension and the development of surgical treatment plan.
