The authors investigated the relationship between Tamm-Horsfall protein (THP) and the immune system and the mechanism of THP induced tubulointerstitial nephritis (TIN) through the autoimmune response from the perspective of mononuclear and lymphokine. The results of a series of studies suggest that under physiological conditions. THP may be involved in the kidney’s clearance and metabolism of certain cytokines, such as interleukin-1, interleukin-2 and tumor necrosis factor. When certain pathological conditions cause THP to directly enter the renal interstitium, TIN may be induced by stimulating the cellular immune response. Monocytes / macrophages and certain monokines secreted by them are the key factors mediating the inflammation and fibrosis of such TIN tissues. This conclusion not only helps to further clarify the pathophysiological role of THP. It may also give new insights into the treatment of certain TINs.
