目的了解妇科门诊患者HPV感染率、亚型分布以及不同年龄HPV感染状况,为防治HPV感染提供科学依据。方法对2008年10月~2011年7月来该院妇科门诊就诊的11 461例患者进行登记,并采集其阴道、宫颈分泌物作为标本,用导流杂交基因芯片技术进行21种HPV基因型的分型检测。用EXCEl 2007和SPSS 17.0软件建立数据库及统计分析,HPV感染率比较采用统计学χ2检验。结果①HPV阳性总检出率为44.45%(5 094/11 461),高危型别感染率为40.54%(4 646/11 461),低危型别感染率为4.26%(488/11 461),单一型别感染率为30.89%(3 540/11 461),多重型别感染率为14.00%(1 604/11 461);②HPV感染率最高的亚型为HPV16型(15.55%),且以单一感染和双重感染为主;③不同年龄检查者的HPV感染率差异有统计学意义(χ2=77.03,P<0.01),随着年龄的增加,HPV的感染率逐渐增加(趋势性χ2=76.67,P<0.01),不同年龄的HPV感染者感染不同的高低亚型构成上有统计学意义(χ2=121.70,P<0.01),不同年龄的HPV感染者单一及多重感染构成上也具有统计学意义(χ2=121.70,P<0.01),各年龄HPV感染者均以单一及高危感染为主,但在30岁以下及60岁以上年龄HPV感染者中高低危合并多重感染高于其他年龄段。结论该院妇科门诊患者HPV感染率处于较高水平,HPV感染率最高的亚型为HPV16。不同年龄检查者HPV感染率不同,且随年龄增加有逐渐增加趋势,高低危亚型、单一及多重感染的构成在不同的年龄中有差别,30岁以下及60岁以上年龄女性中,高低危合并的多重感染高于30~60岁的女性。
Objective To understand the prevalence of HPV infection, subtypes and HPV infection in gynecology outpatients so as to provide a scientific basis for the prevention and treatment of HPV infection. Methods A total of 11 461 patients who came to the gynecology clinic from October 2008 to July 2011 were enrolled in this study. Their vaginal and cervical secretions were collected for specimen collection. 21 kinds of HPV genotypes Typing test. Using EXCEl 2007 and SPSS 17.0 software to establish a database and statistical analysis, HPV infection rates were compared using the statistical χ2 test. Results ① The positive rate of HPV positive was 44.45% (5 094/11 461), the high-risk type infection rate was 40.54% (4 646/11 461), the low-risk type infection rate was 4.26% (488/11 461) The single-type infection rate was 30.89% (3 540/11 461) and the multiple-type infection rate was 14.00% (1 604/11 461). ② The highest HPV subtype was HPV16 type (15.55%), (Χ2 = 77.03, P <0.01). With the increase of age, the infection rate of HPV increased gradually (trend χ2 = 76.67, P <0.01). There were statistically significant differences in the infection of different subtypes of HPV between different age groups (χ2 = 121.70, P <0.01). The single and multiple infections of HPV infection of different ages were also statistically significant (χ2 = 121.70, P <0.01). HPV infection was mainly single and high risk in all age groups. However, HPV infection in those under 30 years of age and above 60 years of age was higher than those in other age groups. Conclusion The prevalence of HPV infection in gynecology outpatient department of this hospital is at a high level, and the highest HPV subtype is HPV16. HPV infection rates of different age examiners were different, and increased gradually with age, high and low risk subtypes, single and multiple infections in different age groups have different ages, 30 years of age and above the age of 60 women, high and low risk Multiple combined infections were higher in women aged 30 to 60 years.
