目的 观察乙炔雌二醇用药前后原发性闭经患者骨代谢的反应性,为该类患者骨质疏松症的防治提供理论依据。方法 选择10 例低雌激素性原发性闭经患者给予乙炔雌二醇0-05 mg/d,连续给药3 个周期。并于用药前后分别测定空腹尿钙与肌酐(Ca/Cr)、羟脯氨酸与肌酐(OHPr/Cr) 比值以及血清雌二醇(E2) 和降钙素(CT) 的水平。结果 原发性闭经患者乙炔雌二醇给药前后血清CT水平升高,空腹Ca/Cr、OHPr/Cr 比值显著下降,而血清ALP的水平尚未出现显著性变化。结论 乙炔雌二醇用药可显著升高年轻低雌激素性原发性闭经患者血清CT的水平,明显抑制骨钙的释放和骨基质胶原的分解,从而有效防止该类患者骨量的丢失。
Objective To observe the reactivity of bone metabolism in patients with primary amenorrhea before and after treatment with ethinyl estradiol to provide theoretical basis for the prevention and treatment of osteoporosis in these patients. Methods Ten patients with low estrogen-induced primary amenorrhea were given ethinyl estradiol (0-05 mg / d) for 3 cycles. The levels of fasting urine calcium and creatinine (Ca / Cr), hydroxyproline and creatinine (OHPr / Cr), serum estradiol (E2) and calcitonin (CT) were measured before and after treatment. Results In patients with primary amenorrhea, serum CT levels were elevated before and after administration of ethinyl estradiol. The ratios of fasting Ca / Cr and OHPr / Cr decreased significantly, while serum ALP level did not change significantly. Conclusions Ethinyl estradiol can significantly elevate the level of serum CT in young patients with low estrogen-induced primary amenorrhea, and significantly inhibit the release of bone calcium and the decomposition of bone matrix collagen, thus effectively preventing the loss of bone mass in these patients.
