探讨能谱CT成像技术对鼻咽癌的诊断价值。方法:收集经鼻咽活检病理证实的93例鼻咽部病变患者(其中38例鼻咽癌为实验组,55例鼻咽炎症为对照组)。采用能谱CT对所有患者进行GSI单能模式增强扫描,测量并计算鼻咽部组织的单能量CT值(40ke V~100ke V)及能谱曲线斜率。结果:实验组与对照组的单能量CT值分别为,动脉期:113.15±18.14、95.42±15.25、81.28±12.38、69.48±15.19、58.59±10.17、41.25±9.64、33.41±8.57及71.18±18.12、62.41±15.10、55.17±12.64、48.36±11.96、40.51±10.37、35.48±9.68、29.52±8.49;静脉期:150.39±23.69、138.19±18.65、116.78±15.41、96.51±12.39、81.41±9.37、68.65±10.84、45.38±10.17及90.45±17.22、75.16±13.30、62.37±10.14、50.76±10.6、41.31±9.17、31.68±8.58、24.61±10.19;能谱曲线斜率分别为,动脉期:-0.912±0.16、-0.752±0.15;静脉期:-1.587±0.24、-1.021±0.18。实验组与对照组在动脉期和静脉期的单能量CT值及能谱曲线斜率两两比较差异均有统计学意义(P均<0.05)。结论:能谱CT对鼻咽癌的诊断具有较大的临床价值。
To explore the diagnostic value of energy spectrum CT imaging for nasopharyngeal carcinoma. Methods: Ninety-three patients with nasopharyngeal lesions confirmed by nasopharyngeal biopsy were enrolled. Among them, 38 cases of nasopharyngeal carcinoma were experimental group and 55 cases of nasopharyngeal inflammation as control group. All the patients underwent GSI unipolar mode enhanced scan with energy spectrum CT, and the single-energy CT value (40ke V ~ 100ke V) and the slope of the energy spectrum curve of the nasopharyngeal tissue were measured and calculated. Results: The single-energy CT values of the experimental group and the control group were: arterial phase: 113.15 ± 18.14, 95.42 ± 15.25, 81.28 ± 12.38, 69.48 ± 15.19, 58.59 ± 10.17, 41.25 ± 9.64, 33.41 ± 8.57 and 71.18 ± 18.12, 62.41 ± 15.10,55.17 ± 12.64,48.36 ± 11.96,40.51 ± 10.37,35.48 ± 9.68,29.52 ± 8.49; Vein: 150.39 ± 23.69,138.19 ± 18.65,116.78 ± 15.41,96.51 ± 12.39,81.41 ± 9.37,68.65 ± 10.84 , 45.38 ± 10.17 and 90.45 ± 17.22, 75.16 ± 13.30, 62.37 ± 10.14, 50.76 ± 10.6, 41.31 ± 9.17, 31.68 ± 8.58 and 24.61 ± 10.19, respectively. The slope of the energy spectrum curve was: arterial phase: -0.912 ± 0.16, -0.752 ± 0.15; venous phase: -1.587 ± 0.24, -1.021 ± 0.18. There were significant differences between the experimental group and the control group in the single-energy CT value and the slope of the energy spectrum curve at arterial phase and venous phase (P <0.05). Conclusion: Spectrum CT has great clinical value in the diagnosis of nasopharyngeal carcinoma.
