目的:应用二维斑点追踪成像(speckle tracking imaging,STI)联合实时三维超声(real time three dimensional echocardiography,RT3DE)探讨系统性红斑狼疮(systemic lupus erythematosus,SLE)患者左心房(左房)早期功能的变化。方法:对40例无心脏受累临床表现的SLE患者(SLE组)和38例健康体检者(对照组)行超声心动图检查并存储图像,应用STI技术脱机测量并对左房前壁、后壁、侧壁和房间隔的应变曲线进行分析,获得收缩期、舒张早期和舒张晚期各房壁平均应变率(m SRs、m SRe和m SRa)。采用RT3DE测定左房主动射血分数(LAAEF)、左房被动射血分数(LAPEF)和左房膨胀指数(AEI),分析LAAEF与SRa、LAPEF与SRe及AEI与SRs之间的相关性。结果 :1SLE组和对照组二尖瓣环舒张早期速度分别为(0.67±0.12)m/s和(0.72±0.11)m/s(P>0.05);2SLE组和对照组的m SRs为(3.81±1.02)/s和(4.09±1.13)/s(P>0.05),m SRe为(-3.68±1.70)/s和(-4.11±0.79)/s(P<0.05),m SRa为(-3.73±1.56)/s和(-2.89±0.79)/s(P<0.01);3SLE组和对照组的LAAEF为(48.0±8.9)%和(33.0±7.4)%(P<0.01),LAPEF为(37.1±8.2)%和(49.5±7.4)%(P<0.01),AEI为(85.7±27.5)%和(95.3±22.5)%(P=0.17);4LAAEF和SRa(r=0.816,P<0.01)、LAPEF和SRe(r=0.786,P<0.01)及AEI和SRs(r=0.756,P<0.01)均具有较好的相关性。结论:SLE患者早期左房功能改变,表现为管道功能明显受损,储存功能降低和辅助泵功能增强。
Objective: To investigate the early function of left atrium (left atrium) in patients with systemic lupus erythematosus (SLE) using two-dimensional speckle tracking imaging (STI) and real-time three dimensional echocardiography Variety. Methods: Echocardiography was performed in 40 patients with SLE without heart involvement (SLE group) and 38 healthy controls (control group). The images were taken and measured by STI technique. The anterior wall of the left atrium, The strain curves of the wall, the lateral wall and the atrial septum were analyzed to obtain the average strain rates (m SRs, m SRe and m SRa) in systolic, early diastolic and late diastolic wall. The correlation between LAAEF, SRa, LAPEF, SRe, AEI and SRs was analyzed by RT3DE, LAAEF, LAPEF and AEI. Results: The early diastolic velocities of mitral annulus were (0.67 ± 0.12) m / s and (0.72 ± 0.11) m / s respectively in the 1SLE group and the control group (P> 0.05) MSRe was (-3.68 ± 1.70) / s and (-4.11 ± 0.79) / s respectively (P <0.05), m SRa was (-) 1.0 ± 1.02 / s and (4.09 ± 1.13) / s 3.73 ± 1.56 / s and -2.89 ± 0.79 / s, respectively (P <0.01). LAAEF was (48.0 ± 8.9)% and (33.0 ± 7.4)% respectively in the 3SLE group and the control group ALE was (85.7 ± 27.5)% and (95.3 ± 22.5)% respectively (P = 0.17); 4LAAEF and SRa (r = 0.816, P < 0.01). There was a good correlation between LAPEF and SRe (r = 0.786, P <0.01) and AEI and SRs (r = 0.756, P <0.01). CONCLUSIONS: Early left atrial function changes in patients with SLE manifest as impaired duct function, reduced storage, and enhanced ancillary pump function.
