1989年2月4日,陕西秦岭发电厂失火,参加灭火的工人田某,使用手提式干粉灭火器时,因灭火器喷不出粉,突然爆炸,下封头从与简体对接的焊缝处断开飞出,击中胸部倒地,致使右胸多发性肋骨骨折,造成气胸、血胸,经抢救无效死亡。分析这次事故的原因主要是: (1)灭火器制造质量低劣。灭火器下封头与筒身对接的焊缝,焊透部分只占全部焊缝的9.08%。 (2)干粉结块。事后检查发现,灭火器出粉管被坚硬的粉柱堵塞(造成结块堵塞原因不明),致使筒体充压后无处释放。如果焊接质量合格,即使出粉管堵塞,灭火器也不至于爆破。 (3)灭火器的维修不认真。这台灭火器曾送西安某灭火器维修单位装修,重新装了
