目的:与常规门静脉造影对照,评价门静脉三维动态增强磁共振血管成像(3dimensionaldynamiccontrastenhancedMRA,3DDCEMRA)显示门静脉开放性及侧支循环的能力。材料与方法:共有20例患者做了门静脉3DDCEMRA成像,分析门静脉3DDCEMRA上门脉主干、肝内门脉左右分支的开放性;侧支循环发生的部位和严重程度。3DDCEMRA的显示结果与门静脉造影术(经皮肝穿门脉造影或经脾动脉/肠系膜上动脉间接门脉造影)相对照,评价3DDCEMRA与门静脉造影术的符合情况。结果:在20例患者60支门脉主干和左右分支中,有57支两种检查方法显示门脉的开放性的结果相同,符合率为95%。但有3支肝内门脉分支,两种检查方法评判其开放性所获结果不一致。3DDCEMRA所揭示的侧支循环的分布情况则同门静脉造影术完全一致,符合率为100%。结论:门静脉3DDCEMRA同常规门静脉造影术符合率很高,它能较准确地显示门脉的开放性和侧支循环的分布情况
OBJECTIVE: To compare the portal venous angiography with conventional portal venous angiography to evaluate the portal venous openness and collateral circulation in 3-dimensional dynamic contrast enhanced 3D MRA (3DDCEMRA). Materials and Methods: A total of 20 patients underwent 3DDCEMRA imaging of the portal vein to analyze the openness of the portal vein in the portal vein and the left and right branches of the portal vein in the portal vein. The location and severity of collateral circulation occurred. The results of 3DDCEMRA were compared with portal venography (percutaneous transhepatic portography or splenic / superior mesenteric artery portal angiography) to evaluate the compliance of 3DDCEMRA with portal vein angiography. RESULTS: Of the 60 branches of the portal vein and the left and right branches of the 20 patients, 57 showed the same result of openness of the portal vein with a coincidence rate of 95%. However, there are 3 branches of intrahepatic portal vein, and the results of the two tests are inconsistent. The distribution of collateral circulation revealed by 3DDCEMRA is exactly the same as that of portal vein angiography, with a coincidence rate of 100%. Conclusion: The coincidence rate of portal vein 3DDCEMRA and conventional portal venography is very high, which can show the distribution of portal opening and collateral circulation more accurately
