目的探讨叶绿素在妇科恶性肿瘤淋巴清扫术中的应用价值。方法将14支狗的睾丸或子宫角处注射叶绿素溶液,注射后2,4,6,8,12,24,48,72,120h后分别取其腰淋巴结作光、电镜观察,并按相同时间取股静脉血作叶绿素测定。对23例宫颈癌患者,术前随机分为宫颈注射叶绿素组13例和对照组10例。术中观察记录盆腔淋巴结的染色摘除情况,并取淋巴结作组织切片,显微镜观察。结果叶绿素注射4h后,其在血液中的浓度最高为360.65μg/ml,24h后,血液测不到叶绿素。狗腰淋巴结平均显色时间为8min,显色可持需120h以上。宫颈癌病人淋巴结染色组,盆腔淋巴结平均摘除31.38个,对照组22.5个(P<0.01);染色组所用时间为57min,对照组为74min。结论宫颈注射叶绿素显示盆腔淋巴结,可提高淋巴结清除率并能缩短手术时间
Objective To investigate the application value of chlorophyll in lymphatic dissection of gynecologic malignancies. Methods The chlorophyll solution was injected into the testicles or uterine horns of 14 dogs. The lymph nodes were taken for light and electron microscopy after 2, 4, 6, 8, 12, 24, 48, 72 and 120 hours after injection. Take venous blood for chlorophyll determination. 23 patients with cervical cancer were randomly divided into the chlorophyll injection group and the control group. Intraoperative observations recorded pelvic lymph node staining removal, and lymph nodes were taken for histological section observation. Results After chlorophyll injection for 4 h, the highest concentration in the blood was 360.65 μg/ml. After 24 hours, the blood could not detect chlorophyll. The mean time for coloring of the dog’s lumbar lymph nodes was 8 minutes, and the color development could take more than 120 hours. In the lymph node staining group of cervical cancer patients, 31.38 pelvic lymph nodes were removed and 22.5 in the control group (P<0.01). The time for the staining group was 57 minutes and that for the control group was 74 minutes. Conclusion Cervical injection of chlorophyll shows pelvic lymph nodes, can increase lymph node clearance rate and shorten the operation time
