目的探讨经阴道彩色多普勒超声(TV-CDS)在控制性超排卵(COH)启动时评估卵巢储备功能及预测卵巢反应性的价值。方法选取本中心接受体外受精-胚胎移植(IVF-ET)治疗的186例不孕患者,采用TV-CDS检测促性腺激素(Gn)启动日卵巢大小、窦卵泡数(AFC)、子宫、卵巢动脉血流参数,根据获卵数及人绒毛膜促性腺激素(HCG)日E2水平分为卵巢低反应组和非卵巢低反应组,比较两组间各指标的差异。结果两组间AFC、卵巢内血流评分、卵巢间质动脉收缩期峰值速度(OA-PSV)、卵巢间质动脉收缩期阻力指数(OA-RI)、子宫动脉收缩期阻力指数(UA-RI)差异有统计学意义(P<0.05)。获卵数与AFC、卵巢内血流评分、OA-PSV呈正相关关系,与UA-RI、OA-RI呈负相关关系,其中以卵巢内血流评分、OA-PSV、AFC及OA-RI的相关程度较高。ROC曲线下面积预测卵巢低反应中,UA-PSV的AUC值对卵巢储备功能预测能力最高,其次是AFC。结论 Gn启动日TV-CDS检测AFC、卵巢内血流评分、OA-PSV、OA-RI、UA-RI可作为评估于卵巢储备功能及预测卵巢反应性的有效方法。
Objective To investigate the value of transvaginal color Doppler ultrasound (TV-CDS) in evaluating ovarian reserve and predicting ovarian response during the initiation of controlled ovarian hyperstimulation (COH). Methods 186 cases of infertility treated by IVF-ET in our center were enrolled in this study. The ovarian size, antral follicle count (AFC), uterine and ovarian artery Blood flow parameters, according to the number of oocytes and human chorionic gonadotropin (HCG) day E2 levels were divided into low-response ovarian response group and non-ovarian response group, the differences between the two groups were compared. Results The AFC, blood flow in ovary, OA-PSV, OA-RI, UA-RI of uterine artery ) Difference was statistically significant (P <0.05). The number of oocytes retrieved was positively correlated with AFC, ovarian blood flow score and OA-PSV, but negatively correlated with UA-RI and OA-RI. Among them, ovarian flow score, OA-PSV, AFC and OA-RI A higher degree of correlation. In the area under the ROC curve for prediction of ovarian low response, the AUC value of UA-PSV had the highest ability to predict ovarian reserve function, followed by AFC. Conclusion TV-CDS detection AFC, intra-ovarian blood flow score, OA-PSV, OA-RI and UA-RI can be used as an effective method to assess ovarian reserve function and predict ovarian response.
