目的 探讨P波离散度 (Pd)预测原发性高血压 (EH)伴阵发性心房颤动 (Af)的价值。方法 对 36例EH伴阵发性Af者 (A组 )、30例EH无Af者 (B组 )的Pd进行比较。结果 A组Pd为 (46 4± 13 8)ms ,与B组 (对照组 ) (2 8 7± 10 2 )ms相比明显延长 (P <0 0 1)。为预测Af,取P波时限≥ 12 0ms时 ,敏感性为 84 %、特异性为 78% ,阳性预测准确性为 86 %。当Pd≥ 4 0ms时 ,敏感性为 87%、特异性为 91% ,阳性预测准确性为 89%。结论 Pd增加和P波宽度延长 ,可预测EH并发阵发性Af的危险性
Objective To investigate the value of P wave dispersion (Pd) in predicting essential hypertension (EH) with paroxysmal atrial fibrillation (Af). Methods 36 cases of EH with paroxysmal Af (group A) and 30 cases of EH without Af (group B) Pd were compared. Results Pd in group A was (46 4 ± 13 8) ms, which was significantly longer than that in group B (28 8 ± 10 2 ms) (P 0 01). To predict Af, the P-wave duration was ≥84 ms with a sensitivity of 84%, a specificity of 78%, and a positive predictive accuracy of 86%. When Pd ≥ 4 0ms, the sensitivity was 87%, the specificity was 91%, and the positive predictive accuracy was 89%. Conclusions The increase of Pd and the prolongation of P wave width can predict the risk of paroxysmal Af in EH
