腹股沟内或腹内未降睾丸常因精索血管蒂过短而使一期睾丸阴囊固定术成为一个外科棘手问题。精索血管高位离断术则较好的解决了在这种情况下睾丸阴囊固定的手术难题,但有关术后睾丸血流、内分泌和生精功能方面的影响,尚值得我们研究。本文曾对幼年大鼠作了精索血管腹内离断的有关实验,待幼鼠成年后,对其睾丸血流指数、血清睾酮水平和睾丸组织学方面进行了检测。实验结果:睾丸血流指数在高位离断组与正常对照组间无显著性差异(P<0.05),且这两组都明显高于低位离断组(P<0.01)。HCG刺激前后的术后血清睾酮值高位离断组与正常对照组间均无显著性差异(P>0.05),且这两组都明显高于低位离断组和睾丸切除组(P<0.01),但低位离断组与睾丸切除组间也均无显著性差异(P>0.05)。曲精细管平均直径和睾丸组织学评分在高位离断组与正常对照组间均无显著性差异(P>0.05),且这两组都明显高于低位离断组(P<0.01)。这些结果表明:幼年大鼠精索血管高位离断术后,通过睾丸侧枝循环的代偿,尚能够保证成年大鼠的睾丸有效血流供给,且对成年大鼠的睾丸内分泌和生精功能无明显不利的影响。本研究提示:精索血管高位离断术有可能作为临床小儿高位隐睾治疗的一?
In the groin or intra-abdominal underestimated testicular often due to spermatic vascular pedunculated shortening of a testicular scrotum fixation surgery becomes a thorny issue. High spermatic vascular ablation is a better solution to the surgery in this case of testicular scrotal surgery, but postoperative testicular blood flow, endocrine and spermatogenic function of the impact is still worth our study. In this paper, young rats were made intra-abdominal spermatic vascular rupture of the relevant experiments to be adulthood, the testis blood flow index, serum testosterone levels and testicular histology were tested. Experimental results: There was no significant difference in testis blood flow index between the high disconnection group and the normal control group (P <0.05), and both groups were significantly higher than the low disconnection group (P <0.01). There was no significant difference between the high-dose serum testosterone group and the normal control group before and after HCG stimulation (P> 0.05), and both groups were significantly higher than those in the low-off group and the testicular excision group (P < 0.01). However, there was no significant difference between low disconnection group and orchiectomy group (P> 0.05). Mean diameter of seminiferous tubules and histopathological score of testis showed no significant difference between the high position disconnection group and the normal control group (P> 0.05), and both groups were significantly higher than the low position disconnection group (P <0. 01). These results indicate that the compensatory testis collateral circulation can still provide effective testicular blood flow in adult rats after experimental high blood vessel disconnection in young rats and have no effect on the testicular endocrinology and spermatogenesis in adult rats Obviously adverse effects.
