目的:探讨Cys C联合NGAL在急性肾损伤(AKI)中的评估价值。方法:2011-09-2016-04选择在我院ICU病房进行诊治的39例急性肾损伤患者作为AKI组,同期选择来自我院体检中心进行体检并证实身体健康的正常人39例作为对照组,2组都进行常规肾功能指标、胱抑素C(Cys C)、中性粒细胞明胶酶相关脂质运载蛋白(NGAL)检测与相关性分析。结果:AKI组的尿蛋白定量、血清肌酐与尿素氮值都明显高于对照组(P<0.05)。AKI组的Cys C与NGAL含量分别为(1.15±0.31)mg/L和(25.92±6.29)μg/ml,都明显高于对照组的(0.78±0.29)mg/L和(1.94±0.72)μg/ml(P<0.05)。在AKI组中,Spearman相关分析显示Cys C与NGAL存在明显的正相关性(r=0.442,P<0.05);多因素Logistic回归分析结果显示,NGAL与肌酐水平关系密切(P<0.05),CysC则与尿蛋白定量量关系密切(P<0.05)。结论:Cys C在NGAL在AKI患者中呈现高表达状况,可从一定程度反映AKI患者的病情,从而有利于指导治疗与评估预后。
Objective: To evaluate the value of Cys C combined with NGAL in acute renal injury (AKI). Methods: 39 patients with acute kidney injury who were diagnosed and treated in ICU ward of our hospital were selected as the AKI group. In the same period, 39 normal subjects who were physically examined and confirmed by physical examination in our hospital were selected as the control group, The renal function indexes, Cystatin C (Cys C) and neutrophil gelatinase-associated lipocalin (NGAL) were detected in 2 groups. Results: The proteinuria, serum creatinine and urea nitrogen in AKI group were significantly higher than those in control group (P <0.05). The levels of Cys C and NGAL in AKI group were (1.15 ± 0.31) mg / L and (25.92 ± 6.29) μg / ml respectively, which were significantly higher than those in control group (0.78 ± 0.29 mg / L and 1.94 ± 0.72 μg / ml (P <0.05). In AKI group, Spearman correlation analysis showed that there was a significant positive correlation between Cys C and NGAL (r = 0.442, P <0.05). The multivariate Logistic regression analysis showed that there was a close relationship between NGAL and creatinine (P <0.05) Then with the quantitative relationship between urinary protein (P <0.05). CONCLUSIONS: Cys C is highly expressed in NGAL patients with AKI and may reflect the condition of AKI to a certain extent, which is helpful for guiding the treatment and evaluating the prognosis.
