目的:探讨经颅多普勒在脑动脉瘤术前、术后的应用价值。方法:应用经颅多普勒对96例动脉瘤患者进行术前、术后脑血管血流信号及速度进行监测,所得数据加以分析,从而了解脑血管痉挛的程度与动脉瘤破裂出血后时间的关系以及脑动脉瘤夹闭后脑血管痉挛高峰期和持续的时间。结果:所有患者在动脉瘤破裂后第8~14天脑血流流速达最高峰,以后脑血流流速逐渐减慢,21天后流速接近正常。术后1~7天脑血流流速达最高峰,14天后流速接近正常。结论:经颅多普勒是脑动脉瘤术前、术后监测脑血管痉挛程度最方便、最经济、最有效的方法,对患者的治疗、手术时机及预后都有指导意义。
Objective: To investigate the value of transcranial Doppler before and after cerebral aneurysm surgery. Methods: Transcranial Doppler was used to monitor the signal and velocity of cerebrovascular blood flow in 96 patients with aneurysm before operation and after operation. The data were analyzed to understand the relationship between the degree of cerebral vasospasm and the time after rupture of aneurysm And the peak and duration of cerebral vasospasm after occlusion of cerebral aneurysm. Results: In all patients, the cerebral blood flow velocity peaked on the 8th to the 14th day after ruptured the aneurysm. After that, the cerebral blood flow velocity gradually slowed down and the flow velocity approached normal after 21 days. Cerebral blood flow reached the peak 1 to 7 days after operation, and the flow rate was nearly normal after 14 days. Conclusion: Transcranial Doppler is the most convenient, economical and effective way to monitor the degree of cerebral vasospasm before and after cerebral aneurysm. It has guiding significance for the treatment, operation timing and prognosis of patients.
