目的:探讨IGF-Ⅱ在子痫前期和子痫中的作用、机理,为评估子痫前期及子痫发生、发展和胎儿宫内生长状况提供临床依据和指标。方法:用放免法检测子痫前期和子痫患者90例、正常晚期妊娠孕妇34例母血和脐静脉血IGF-Ⅱ水平。结果:正常妊娠组脐静脉血和母血中IGF-Ⅱ无显著性差异。子痫前期-子痫各组和正常妊娠组母血中IGF-Ⅱ无显著性差异。子痫前期-子痫组脐静脉血中IGF-Ⅱ水平明显低于正常妊娠组。子痫前期-子痫各组脐静脉血和母血中IGF-Ⅱ有显著性差异。子痫前期-子痫新生儿体重随脐静脉血IGF-Ⅱ水平的减低而降低,呈正相关性。结论:子痫前期和子痫患者的发病及严重程度与脐静脉血中IGF-Ⅱ水平有明显的关系,可以作为评估子痫前期和子痫发生、发展及严重程度的指标。胎儿脐静脉血中IGF-Ⅱ水平与胎儿的生长发育及新生儿出生体重有明显的正相关性,为评估胎儿及新生儿体重提供临床指标。
Objective: To explore the role and mechanism of IGF-Ⅱ in preeclampsia and eclampsia, and to provide a clinical basis and index for evaluating the occurrence and development of preeclampsia and eclampsia and fetal intrauterine growth. Methods: The levels of IGF-Ⅱ in maternal blood and umbilical vein of 90 pregnant women with preeclampsia and eclampsia and 34 pregnant women with normal late pregnancy were detected by radioimmunoassay. Results: There was no significant difference in IGF-Ⅱ between umbilical cord blood and maternal blood in normal pregnancy group. There was no significant difference of IGF-Ⅱ in maternal blood between preeclampsia-eclampsia group and normal pregnancy group. The levels of IGF-Ⅱ in umbilical vein blood in preeclampsia-eclampsia group were significantly lower than those in normal pregnancy group. Preeclampsia - eclampsia in each group umbilical cord blood and maternal blood IGF-Ⅱ significant difference. The body weight of preeclampsia-eclampsia decreased with the decrease of IGF-Ⅱ in umbilical cord blood, which was positively correlated. Conclusion: The incidence and severity of preeclampsia and eclampsia are closely related to the level of IGF-Ⅱ in umbilical vein blood and can be used as an index to evaluate the occurrence, development and severity of preeclampsia and eclampsia. Fetal umbilical vein blood IGF-Ⅱ levels and fetal growth and birth weight of newborns have a significant positive correlation, in order to assess the fetus and neonatal weight to provide clinical indicators.
