目的探讨自动乳腺全容积扫查(ABVS)与常规手持超声(HHUS)检测和诊断乳腺病灶的一致性。方法回顾性分析因乳腺疾病来我院就诊患者超声检查资料,对HHUS和ABVS发现的乳腺病灶数目、大小、定位、声像图特征及最终分类进行一致性分析。结果乳腺患者90例,HHUS发现病灶138个,ABVS发现病灶148个,两种方法共同发现病灶113个,组内相关系数0.870。两种方法病灶时钟位置、距乳头距离、距皮肤距离一致性分别为0.684、0.241、0.772,病灶上下径、左右径及前后径一致性参数分别为0.625、0.610、0.511。两种方法声像图特征描述的一致性参数分别为外形(0.554)、纵横比(0.617)、边界(0.722)、内部回声(0.392)、后方回声特征(0.177)、微钙化(0.130)。病灶BI-RADS分类一致性参数为0.539。结论在乳腺病灶检测和诊断方面,ABVS同HHUS有着较好的一致性,可作为常规超声的补充方法应用于乳腺疾病的筛查、随访及新辅助化疗疗效评价等方面。
Objective To investigate the consistency of automatic breast volume scanning (ABVS) and conventional hand-held ultrasound (HHUS) in detecting and diagnosing breast lesions. Methods We retrospectively analyzed the data of ultrasonographic examination of patients from our hospital due to breast disease and analyzed the consistency, number, size, location, sonographic features and final classification of breast lesions detected by HHUS and ABVS. Results 90 cases of breast cancer, HHUS found 138 lesions, ABVS found 148 lesions, the two methods together found 113 lesions, the group correlation coefficient of 0.870. The two methods of lesion clock location, distance from the nipple, the distance from the skin consistency were 0.684,0.241,0.772, upper and lower diameter of the lesion, left and right diameter and anteroposterior diameter consistency parameters were 0.625,0.610,0.511. The conformal parameters described by the two methods were contour (0.554), aspect ratio (0.617), boundary (0.722), internal echo (0.392), posterior echo characteristic (0.177), and microcalcification (0.130). The BI-RADS classification conformability parameter was 0.539. Conclusion In the detection and diagnosis of breast lesions, ABVS has a good consistency with HHUS, which can be used as a supplementary method of routine ultrasound in the screening of breast diseases, follow-up and evaluation of the efficacy of neoadjuvant chemotherapy.
