①目的在临床状态下验证Hatle公式在功能正常的二尖瓣位双叶机械瓣应用的合理性。②方法利用多普勒超声测量116例接受CarboMedics人工二尖瓣置换病人的二尖瓣瓣口舒张期前向血流和瓣口面积,并与产品标定的瓣口面积相比较。③结果用Hatle公式估测的瓣口面积和其标定的瓣口面积之间无显著相关性(r=0.23,P>0.05);压差半降时间与舒张期二尖瓣瓣口最大流速(r=0.77,P<0.001)、R-R间期(r=0.70,P<0.01)、舒张期充盈时间(r=0.68,P<0.01)、二尖瓣瓣口流率(r=0.55,P<0.05)及每搏量(r=0.50,P<0.05)均存在较密切的关系,而与瓣口面积存在极差的相关性(r=0.09,P>0.05)。④结论在功能正常的二尖瓣位双叶机械瓣,压差半降时间更多地受到瓣膜外因素而不是瓣口面积的影响,Hatle公式不适宜应用于此种状态下
Objective To validate the validity of the Hatle’s formula in the clinical application of mitral valve bicomponent mechanical valve. ② Methods Doppler echocardiography was used to measure the antegrade diastolic anterograde flow and orifice area in 116 patients undergoing CarboMedics artificial mitral valve replacement and compared with the calibrated valve area. Results There was no significant correlation between the port area estimated by Hatle’s formula and the calibrated valve area (r = 0.23, P> 0.05). The half-pressure difference time and diastolic mitral valve (R = 0.77, P <0.001), R-R interval (r = 0.70, P <0.01) and diastolic filling time .01), mitral valve orifice flow rate (r = 0.55, P <0.05) and stroke volume (r = 0.50, P <0.05) There was a very poor correlation between the valve orifice area (r = 0.09, P> 0.05). ④ Conclusion In the normal function of the mitral valve double-leaf mechanical valve, the pressure drop half-time is more affected by the extravalural factors rather than the valve area, Hatle’s formula is not suitable for this state
