目的探讨不同孕周的正常胎儿DV的阻力指数(DVI)、搏动指数(PIV)及静脉峰值流速指数(PVIV)的参数变化,建立胎儿DV的DVI、PIV及PVIV的正常值范围。方法选取不同孕周的472例孕妇,胎儿正常,按孕周分为7组。行胎儿静脉导管检查,留取静脉导管彩色多普勒图像及其频谱测量图像,分析静脉导管频谱,获取静脉导管参数。所有数据采用SPSS 19.0软件行统计学分析。结果正常胎儿静脉导管DVI、PVIV及PIV与孕周关系的曲线回归方程:DVI:y=exp(-1.103+9.857/x),(r=0.733,P<0.01);PVIV:y=exp(-0.996+10.086/x),(r=0.694,P<0.01);PIV:y=exp(-0.953+10.540/x),(r=0.696,P<0.01)。如果不考虑孕周影响,胎儿静脉导管DVI、PVIV、PIV的95%预测范围分别是(0.31,0.75)、(0.33,0.87)、(0.35,0.93)。结论胎儿静脉导管的DVI、PIV及PVIV与孕周关系密切,随着孕周的增加减低,本研究的DVI、PIV及PVIV的95%预测范围可为静脉导管的异常变化提供临床参考。
Objective To investigate the changes of parameters of DVI, PIV and PVIV in normal fetus at different gestational weeks to establish the normal range of DVI, PIV and PVIV for fetal DV. Methods 472 pregnant women of different gestational age were selected. The fetus was normal and divided into 7 groups according to gestational age. Intravenous catheterization of fetal catheters, venous catheter color Doppler images and their spectral measurements were taken, the venous catheter was analyzed for venous catheter parameters. All data using SPSS 19.0 software line statistical analysis. Results The regression equation of DVI, y = exp (-1.103 + 9.857 / x), (r = 0.733, P <0.01) for normal fetal venous catheters, PVIV: 0.996 + 10.086 / x), (r = 0.694, P <0.01); PIV: y = exp (-0.953 + 10.540 / x), (r = 0.696, P <0.01). The 95% predicted range of DVI, PVIV and PIV for fetal intravenous catheters were (0.31,0.75), (0.33,0.87) and (0.35,0.93), respectively, irrespective of gestational age. Conclusions DVI, PIV and PVIV in fetal venous catheters are closely related to gestational age and decrease with gestational age. The 95% predicted range of DVI, PIV and PVIV in this study can provide clinical reference for the abnormal changes of venous catheters.
