目的观察微波热毁损治疗肝脏海绵状血管瘤的疗效及安全性。方法 21例肝脏海绵状血管瘤患者,在B超引导下采用微波固化治疗肝脏单发及多发海绵状血管瘤5例,常规手术探查肝脏,对瘤体较大(>10cm)、距体表位置较远或多发难以切除的16例患者,采用术中微波固化技术治疗。结果全组术后随访3~28个月,18例患者(23个病灶),瘤体体积缩小80%以上,瘤体血供全部消失,病灶完全毁损;3例患者(4个病灶)瘤体体积缩小40%以上,病灶内血流明显减少。术中及术后的并发症有少量出血、发热、肝功能轻度受损,对症治疗后均痊愈,无手术死亡。结论微波固化治疗肝海绵状血管瘤,手术操作简单,安全有效,并发症少。
Objective To observe the curative effect and safety of microwave thermal damage in treating cavernous hemangioma of the liver. Methods Twenty-one patients with hepatic cavernous hemangioma underwent ultrasound-guided microwave-assisted curing of single and multiple cavernous hemangiomas in the liver. The liver was routinely surgically examined and the size of the tumor was larger (> 10 cm) More or more difficult to remove 16 cases of patients, the use of intraoperative microwave curing technology. Results All the patients were followed up for 3 to 28 months. Among the 18 patients (23 lesions), the volume of the tumor was reduced by 80% or more. The blood supply of the tumor disappeared completely and the lesions were completely destroyed. The tumors of 3 patients (4 lesions) Volume reduced more than 40%, significantly reduced blood flow within the lesion. A small amount of intraoperative and postoperative complications of bleeding, fever, mild liver damage, symptomatic treatment were cured, no operative death. Conclusion Microwave curing of hepatic cavernous hemangiomas is simple, safe and effective, with few complications.
